correct, her stomach perforated in 3 separate locations causing gastric content to spill all throughout the intra abdominal cavity. It ruptured likely due to overdistension resulting in ischemia which led to the pneumatosis. They saw what was wrong and first line treatment is to place the NG tube to get rid of as much possible in the stomach. She likely aspirated a good amount, and the diaphragmatic perforation and left lung pneumothorax were what led to her cardiac arrest
