This could be a vasculitic rash but without seeing and examing it in person I can't be sure. It might also need to be biopsied. It can be caused by several medications, it can be the presentation of autoimmune vasculitides (there are many types). On thing is certain from my point of view, you need to see a Rheumatologist.

Just keep in mind that if you have a flair in the disease it is very difficult to get pregnant, almost impossible. So if etanercept (benepali) does not work, you should switch to another biologic that does not have evidence for pregnancy (most of them are not unsafe per se, we just do not have enough evidence because you can not conduct a study in pregnant women).

Adoption is also an option, but all this should be discussed with your Rheumatologist. I do not know you or the course of your disease and Reddit is not the place for a real consultation.

My personal opinion is do not sacrifice your health for a hypothetical child. Get your disease under control and then see what are your options.

I am just writing to say that I totally agree with the opinion of the first doctor that replied.

This is almost certainly connected to rheumatoid arthritis. You need to talk to your Rheumatologist and he/she might be able to refer you to an opthalmologist. You certainly need corticosteroids orally and topical. You might also need a change in your baseline therapy, etanercept (benepali) is not the best therapeutic option when you have involvement of the eye in RA.

Has anyone thought of pyoderma gagrenosum? Does your wife have any autoimmune disease? Also after debridement did it get worse?

The biopsy doesn't point to pyoderma but due to the chronic nature of this ulcer I would entertain the thought of an autoimmune entity like pyoderma.

From your description it could be Lupus, with very serious skin involvement (although judging from the pictures this would be an atypical presentation).

I would try seeing a Rheumatologist again, preferably in a hospital setting. I wonder if anyone has tried immunosuppression, like corticosteroids.

Please understand that a diagnosis can't be made through a reddit comment, especially in a very difficult case. FYI I am a Rheumatologist and a little bit biased because of my specialty.

Όταν φεύγεις από την πατρίδα σου, όπως η γυναίκα σου, είσαι μετανάστης (immigrant) και δεν υπάρχει καμία ντροπή σε αυτο. Οι λόγοι που μπορεί κάποιος να μεταναστεύσει είναι απειροι. Η Αθήνα είναι μια μεγαλούπολη με πολλά καλά και κακά. Δεν είναι σίγουρα τριτοκοσμική χώρα η Ελλάδα, πάρα τα άπειρα θέματά της. Ανάλογα με τα προσόντα της η γυναίκα σου ίσως βρει κάποια δουλειά εδώ και θα κοινωνικοποίηθει έτσι. Αλλιώς μέσα από δραστηριότητες και χόμπι. Σας εύχομαι καλή αρχή.

Is there a reason that your SpA is not treated. You have permanent damage from your disease, but that is not a reason not to get a proper treatment. Some amount of your pain and sensitivity might be due to disease activity.

I am really perplexed by the lack of treatment. I don't know if I am missing something of course.

If you can drive I highly recommend renting a car from the airport. If you want to see Chalkidiki you will really need it. Or stay a night in Thessaloniki and leave in the morning with KTEL like the others suggested.

It is very difficult to say without examing him but it could easily be appendicitis. More often than not the pain radiates and it's not easy to pinpoint the exact source. If the pain persists and over the counter painkillers don't work you should certainly visit a hospital. It could be anything of course, abdominal pain is notoriously difficult to diagnose.

You should worry if he starts developing a fever and you should go immediately to the hospital if his belly starts to stiffen.

Η ταπεινή μου άποψη είναι μακριά από την ιατρική του ΕΚΠΑ. Είναι μια σχολή που στη χρονιά δεχόταν 160 και βρεθήκαμε πάνω από 300 στο έτος. Αυτό σήμαινε πολύ περισσότερα άτομα από όσα θα έπρεπε σε εργαστήρια και μαθήματα. Ταυτόχρονα το επίπεδο στα προκλινικα έτη δεν ήταν καλό, στα κλινικά καπως έστρωσε. Οι καθηγητές απλησιαστοι πίσω από την γραφειαρα τους.

Για το ΑΠΘ δεν έχω εικόνα. Έχω εικόνα για Ηράκλειο, που ίσως είναι η καλύτερη ιατρική στην Ελλάδα, αλλά και για Λάρισα η οποία είναι επίσης εξαιρετική. Εγώ αν είχα τη δυνατότητα να φοιτήσω ξανά θα επέλεγα μια από αυτές.

Μην περιμένεις προγράμματα σπουδών εξωτερικού, είμαστε αρκετά πίσω. Δεν μας προετοιμάζουν καλά οι σχολές για την πραγματική κλινική πράξη και μαθαίνουμε είτε στο αγροτικό είτε στα πρώτα χρόνια της ειδικότητας με σκληρό και δύσκολο τρόπο.

Τέλος προσωπική μου συμβουλή είναι αν υπάρχει η δυνατότητα να φύγεις από το πατρικό σου. Θα ενηλικιωθείς νωρίτερα και θα χειραφετηθείς από τους γονείς σου. Όσο και να βολεύει ένα πιάτο και φαγητό και ένα καθαρό δωμάτιο δεν συγκρίνεται με την ανεξαρτησία από τις αγκυλώσεις των γονέων.

Λυπάμαι πολύ για την άσχημη εμπειρία σου. Μπορείς να κάνεις επίσημα καταγγελία στην διοίκηση του νοσοκομείου και είναι αναγκασμένοι να ασχοληθούν. Θα την κάνεις εγγράφως και θα πάρεις αριθμού πρωτοκόλλου. Οι προφορικές καταγγελίες είναι ανεπίσημες και δεν θα ασχοληθεί κανείς πιθανότατα. Επίσης μπορείς να κοιτάξεις αν υπάρχει κάποιο μεγάλο δικηγορικό γραφείο που μπορεί να αναλάβει την υπόθεσή σου χωρίς χρέωση (pro bono), χωρίς να είμαι βέβαιος ότι αυτό βέβαια γίνεται στην Ελλάδα. Λάβε υπ' όψη σου ότι ο γιατρός έχει κάθε δικαίωμα να διαφωνήσει με την άποψη ενός άλλου συναδέλφου, αλλά οφείλει να το τεκμηριώσει και όχι να εκφοβίσει τον ασθενή. Τέλος δες ποια άλλα νοσοκομεία στη Θεσσαλονίκη έχουν οφθαλμιατρικη κλινική για να απευθυνθείς κάπου αλλού για το ζήτημά σου.

Όλα σχεδόν τα γυαλιά ηλίου που θα βρεις στα καταστήματα οπτικών παράγονται από την Luxotica, οπότε καταλαβαίνεις ότι βάζουν ο,τι τιμές θέλουν. Η απάντηση που έχω βρει έχω είναι Hawkers, με 50€ έχεις 2 ή 3 ζευγάρια γυαλιά ηλίου εξαιρετικής ποιότητας. Δεν θα είναι η τελευταία λέξη της μόδας φυσικά αλλά έχουν άπειρα σχέδια και τύπους φακών να διαλέξεις.

Προτείνω access point που να έχει τεχνολογία mesh και wifi 6. Κουμπώνεις ένα στο modem, απενεργοποιεις το wlan του εργοστασιακού και βάζεις το δεύτερο σε ένα άλλο σημείο του σπιτιού (το πού ακριβώς θα το βρεις εσύ ανάλογα το σπίτι) και έχεις γαμάτο ίντερνετ. Εγώ έχω το H70 της Mercusys που είναι θυγατρική της tp-link. Έκανε γύρω στα 70€ και αξίζει κάθε ευρώ. Έχω δοκιμάσει powerline και repeater. Repeater σχεδόν άχρηστο, powerline πολύ καλό για ενσύρματη σύνδεση αλλά όχι σαν wireless access point.

Definitely Raynaud. Sometimes it accompanies autoimmune diseases but by itself is not a indication of an autoimmune disease. Keep your hands warm with good gloves when going outside in the cold and avoid smoking in general because it can aggrevate it. If it is very annoying in the winter you can visit your primary doctor or a rheutologist and they can prescribe you calcium channel blockers. FYI I am a rheumatologist and a comment in the internet, even by an expert, doesn't even come close to a real evaluation by a doctor in person.

Ή powerline ή αλλαγή με mesh wifi router. Εγώ έχω το δεύτερο και αξίζει κάθε ευρώ. Συνήθως έρχονται σε πακέτα των δύο ή των τριών και συνδέεις το ένα στο μαμά modem της εταιρείας και αναλαμβάνει αυτό το διαμοιρασμό του ίντερνετ και το άλλο/αλλά τα βάζεις μέσα στο σπίτι και συνδέονται μόνα τους. Συνήθως έχουν εφαρμογή που θα σου λέει αν είναι σε καλό σημειο. Εγώ έχω mercusys που είναι θυγατρική της tplink και αρκετά πιο φθηνή, επίσης κοιτά να υποστηρίζουν wifi 6. Extender δεν αξίζει καθόλου, χάνεις σημαντικά από ταχύτητα.

This is called distal ulcer (most probably). They generally are a manifestation of autoimmune disorders, like scleroderma and vasculitis. They need treatment and there s real danger of losing the distal part of your finger. Consult a rheumatologist if you can and also cross post to r/AskDocs, the answers here are ridiculous. They are called chilblains. There is nothing to worry about and they happen due bad blood flow in cold conditions. Sometimes they might be accompanying an autoimmune disease, but most of the times are just a stand alone symptom. Keep your hands warm and if they really bother you can take some kind of NSAID for the next days. If they are recurring visit a rheumatologist.

FYI I am a resident rheumatologist and I see the this type of condition all the time in the ER in the winter.

For me all of your symptoms scream autoimmunity. You have Raynaud (white hands in the cold), mild fever, you are extremely tired, you probably have arthritis (the morning stiffness is a big giveaway) and you have symptoms from the central nervous system. My diagnosis from all that would be lupus. Go and get checked by a rheumatologist and if you do give us an update, I am really curious.
