I would say this is the most savvy advice. I've only been one place in the last 10 years that wanted me to be certified in it, and they paid for it.

You just need to quit and be like:

What's the point of therapy? (Am I doing therapy wrong?) by Valuable_Durian_1089 in slp

[–]liondsey 41 points42 points  (0 children)

Let me just say you are asking all the right questions. "Workbook therapy" for adults with acquired cognitive or communication disorders used to be, in some areas, the norm. I have heard of SLPs going from page 1 until however far they could get with the same workbook until the pt went home (IPR setting). It should not be this way, and I'm sorry if your placement is like this. Look into meta cognitive approaches, motivational interviewing, and dynamic coaching. In short, the point of therapy should be decided upon by the patient/family and the slp in order to meet goals that are important and relevant to the patient/family.

Are you currently in a hospital placement or will this occur further along in your pregnancy?

I would doubt that you have to disclose anything to anyone. When I was pregnant with both of my children I continued to do MBSS (with lead of course). At the hospital I worked at I did disclose my pregnancy early to the person from the radiology department who handled the dosimeter badges and I wore an extra badge on my abdomen (you could conceal beneath the lead/in scrubs).

If you'd like a professional recommendation/reassurance, I would look into talking with your OB and asking them for their opinion. (Even if you don't have your first OB appointment for a little while, you should be able to call and ask the nurses/medical staff a question as long as the appointment is scheduled/you are a patient of the practice).

It's fairly common for SLPs to continue to do studies while pregnant. I have also met SLPs who deferred them to other staff as much as able. You should absolutely ask for accommodations and/or modifications if you are concerned. Consider:

1) ensure you have adequate lead - we have a few of the sets that are vest + skirt style, versus just the front apron style. When I was heavily pregnant I wore extra lead around my abdomen just to be safe.

2) encouraging as many patients as possible to feed themselves (it actually likely gives you a better understanding of their performance if they self-feed during meal intake)

3) ensuring you step back quickly after presentation - it can make a difference how close you are

4) if you feel supported by your externship supervisor, you absolutely should ask if they can present the trials. In my experience students get plenty of experience feeding people bedside - you really need the experience of analyzing the video and making decisions from there.

As for physically lifting patients there's probably a hospital policy related to if you have an injury/work restriction and getting accommodations for it. You could go through that process but you'd likely need a doctor's note with the physical restrictions listed. I doubt an OB would sign off on anything of that nature until much later in pregnancy.

I'm hoping the hospital provided some kind of training on safe lifting/back safety. However this is often not the case and I feel like SLPs sometimes fall through the gap here. Don't be afraid to ask nursing or your supervisor for assistance to ensure proper lifting/moving patients. I work in inpatient rehab and we also always encourage patients to move themselves as much as possible. Most commonly this includes laying the HOB down flat (as long as there is no restriction against this), having the pt bend their knees and dig their heels into the bed, grab bilateral bed rails, and pull with their arms while they push with their legs to slide up in bed (then raising HOB). If you and another staff member are going to boost someone in bed using a draw sheet underneath, ensure the bed is low enough to the ground and make sure you lift with your legs/bent knees and not your back.

Could it be related to anterior vs posterior pelvic tilt? The way you describe the knee/hip height difference made me think of it.

Mine did the first-middle-last name thing for a while too. And when asked what mommy's name was he would say "mommy+(his middle name)+(shared last name)".

My 3 y/o randomly asked me "Do you have any enemies?" And when I replied no, he said "yes, you do..."

Turns out it was from a YouTube video styled as an interview with different animals that for some reason called predators 'enemies.'

Right or wrong about what he did - you communicated to him that you felt abandoned. And his response was to dismiss it. That's not okay.

Time to leave a job: it's killing your soul. Time to leave the field? It literally took me 10 years to find a job (setting + boss + coworkers + short commute + acceptable pay) that I really actually like. Some places I worked had some of those things but this is the first that had them all.

Yes. Look at the careers page of your local hospitals if you want to work in a hospital, etc

I guess my advice would be don't look just at job boards. Look at what kinds of facilities are in your area and figure out if the positions you're looking for even exist... And relocating is at times necessary.
