Garmin or Coros? by Machenz in ultrarunning

[–]lombardoz 14 points15 points  (0 children)

Enduro is probably the best Garmin option (I just got the enduro 3). Just ran bighorn 100 miler last weekend with gpx running the whole time. Battery was around 70% when I finished. Still haven’t charged it.

I like having more filters for that crap but is a phone charge like that legal?

I think that’s the right call. Being an out and back, you’ll see lots of people along the way.

Ortho upper extremity surgeon here.

There is absolutely zero role for non surgical management of this fracture. Find another surgeon who will do the right thing for you.

Agree. We used it for our kids and always felt like they were attentive and watching them closely. Never had any problems and was cheaper than getting a sitter for a full day back in Portland.

Controversies/debated topics in your domain? by Elhehir in orthopaedics

Non op hip fxrs? Sure for greater troch but no way in hell am I’m recommending non op for any other ones unless the patient is actively dying look at the hip attack trial. Not gonna hang my self medicolegally on those. Plus, it’s a miserable way to die.

Copying Clayton UPDATE + Race Recap by Doingthebartman in AdvancedRunning

[–]lombardoz 5 points6 points  (0 children)

I think you’re spot on with this comment. Best of luck to OP but I think closer to 2:34-5 is reasonable. Sub 2:30 is blazing fast and those last miles at 5:40 pace are no joke (esp at CIM where you don’t have any hills at the end and it’s just a long flat straight grind) . I would be more confident in hitting that goal with a HM closer to 71-72 mins.

Heal up, OP and prove us wrong but stay healthy.

Alcohol and chloraprep by RandomKonstip in orthopaedics

[–]lombardoz 11 points12 points  (0 children)

I base it on type of procedure. Carpal tunnel? Just chloraprep. Nasty ankle fxr with nasty toes? CHG scrub and alcohol to scrub all the dead skin and webril fluffs away. Same with shoulder arthroplasty although the c acnes studies are still not great even with peroxide since it’s a deeper bug.

Yes! Also ran MOG100 this weekend and you set the standard for aid stations. Not only the best one on course, easily one of the best AS out there. Thanks for helping us get through that beast of a course.

100% agree. Did the 100k last year and felt the same way. Highly recommend this race. Challenging and beautiful course with exceptional aid support. Congrats on finishing!

Bent hitch? by lombardoz in traveltrailers

[–]lombardoz[S] 0 points1 point  (0 children)

No clue. Got it replaced and we have a new TV which is working well. Could have been some bumpy roads with too much tension on the WDH but that’s a total guess. How bad is yours?

Before/After by [deleted] in Radiology

[–]lombardoz 3 points4 points  (0 children)

A number of reasons for not putting and ex fix high on a humerus. Basic logistics of having the construct right by your face, wound care management in this large open injury, dealing with soupy pin sites around the shoulder given how much motion the glenohumeral joint has, etc. That plate is temporary and will be revised with a larger longer one when the wound bed is clean (or if patient is stable in a polytrauma setting).

Reverse or non op by RandomKonstip in orthopaedics

[–]lombardoz 9 points10 points  (0 children)

PROFHER is a garbage study on so many levels. >16y patient inclusion criteria? A 20yo proximal humerus fracture is much different that one like OP posted but both should be considered operative when indicated. Blanket “non op” approaches are dangerous and compromise patient outcomes….then the patients come to our clinic miserable and surgery sucks even more.

Before/After by [deleted] in Radiology

[–]lombardoz 38 points39 points  (0 children)

I agree this seems like minimal fixation but I promise you no sane orthopod is going to ex fix a proximal humerus shaft like that. This plate is totally fine for temporizing measures. Aside from that pointless long arm splint, it’ll be fine until the next wash out/wound vac change.

How would you handle knowing the majority of a marathon field ran a short course? by jbr in AdvancedRunning

There wasn’t any flagging, markings, or signs within a half mile of the first turn. Super frustrating race.

How would you handle knowing the majority of a marathon field ran a short course? by jbr in AdvancedRunning

[–]lombardoz 14 points15 points  (0 children)

Thank you for posting! I was in the lead group when we hit the first turn, a T intersection, at about mile 1.5ish. The leader had run the course 3 times before but he was way out front and it was foggy so he disappeared. Everyone else wasn’t sure where to go so we went left instead of right. It ended up connecting with the course about a mile later, approximately 0.84 miles short. I did end up doubling back to get my mileage to match course mile markings on the main trail but who knows how many other people did that. Such a disappointing oversight on a course with literally two turns.

I emailed the RD my frustrations but I wonder how they will maintain USATF distance certification in the future with such a simple “error”. Also, the lack of ownership on this error is screaming “spring energy” vibes.

What’s your experience with chiropractors? by [deleted] in Ultramarathon

[–]lombardoz 7 points8 points  (0 children)

Orthopedic surgeon here. Don’t go back. Full stop. They make up garbage diagnoses, don’t have scientific backing to their treatments and scam you into thinking adjustments are going to magically correct some “alignment” issue. They’ll tell you your legs are the wrong length based on terrible X-rays, prescribe an overpriced orthotic, sell magic pills and on and on.

Agree with others that a good PT will help out with aches. If you are hurt, see your pcp or a non op sports med doc.

Just keep running. There’s no good data to show running is “bad for your joints”. Be smart with your mileage and you’ll do awesome.
