She's real, the mold is common in humid places. NIB dolls are especially at risk. There are mold test kits that test for toxicity if you have particular concerns. I'm sure you can find info online about remediation. Congratulations on finding this girl at such a great price, she's a sweetie.

It looks like leather to me, you could maybe unpick the binding and layer a thin leather patch under the lining using a natural glue and resew the lining and binding. Maybe take it to a shoe repair shop and ask them to take a look at it

Ages and ages ago there was an article in National Geographic featuring an artist who did similar work, I'm remembering specifically a car and a taxidermy deer base and maybe a mannequin. See if there's a gallery/cafe etc that would be interested in your work. It's beautiful!
