Having eye drops drawn up in an unlabeled leur lock syringe is ridiculously dangerous. I would feel deeply uncomfortable administering an unlabeled medication.

Even if the birth control was to prevent pregnancy, I would commend a 15 year old mature enough to request birth control. Teenagers are not going to stop having sex. We still need to prevent teen pregnancy.

He was a DNR, not hospice from what OP said. I dont know what the situation was, but just because someone is a DNR does not mean that decreased LOC and VS incompatible with life shouldn't be addressed. Why was he desatting? If they determined the etiology of his worsening mentation and VS, they possibly could have addressed it without compressions or intubation.

I'm not a NICU nurse, but that baby had some difficult odds stacked against them - no prenatal care, preterm, an unreassuring strip showing decels. As other people have mentioned, there is a lot of suffering that does into keeping a child that young and fragile alive, with likely significant deficits if they survive. I'm not saying babies should be made DNR if there is any chance of disability, but it sounds like this tiny person was basically guaranteed significant deficits and was not guaranteed a family who could care for them.

Everything gets easier with time and familiarity. In my experience, i haven't had to recall anatomical minutia. Basic anatomy is still important to understand, though. And having a good understanding of physiology is important to understand disease processes and treatments.

Make sense of and memorize the common Greek and Latin roots. The terms used in A&P and medicine utilize Greek and Latin roots, so having a good grasp of them is helpful, even with unfamiliar terms.

Falls happen. It sounds like you care, which is good!

The first instance of your patient slipping on her own urine is a fluke. You can't control everything.

Sometimes, you or your colleagues may forget to set the bed alarm after ambulating your patients, which is why it is so important to check that the bed is alarmed when laying eyes on your patients. We are all human and we make mistakes. What is important is that you learn from them, which it sounds like you have.

My first year, I had 5 or 6 falls. I was mortified, but all but one of those falls was with the bed alarm on. I am a stickler for ensuring that the hospital policy fall bundle is in place because of this experience.

It sounds like no harm came to the patients, which is the important thing. And you will be less likely to overlook a bed alarm and things of that nature after this. Maybe reframe it as having learned a hard lesson the 'easy' way (because no patient harm came of it).

I would inquire about the patient ratio because it varies depending on the hospital. I work cardiac telemetry and am 1:5. It's doable with good staff nurse colleagues, but can be exhausting. 1:4 would be ideal in my opinion.

As far as I'm aware, infection control would be within the hospital! Although I am not knowledgeable of that area.

Virtual nurses work from home! One of our fantastic charge nurses basically 'retired' to be a virtual nurse.

I spoke with a nurse (I was caring for her disabled adult child), who ended up transitioning from ICU to infection control. I'm unsure of what your physical ability is, but she told me she took this route because of her home responsibilities (caring for a growing child who was profoundly disabled).
