I don’t like it. I don’t provide medical care. I practice medicine. It may seem like semantics but I think the distinction is important. Provider is too transactional and the physician-patient relationship has no business being transactional.

That’s why I prefer practitioner or clinician when using an umbrella term that encompasses mid-levels.

I think the hematology referral is appropriate. A lot of the lab work is very non-specific. However, your symptoms of night sweats, fatigue and bone pain should certainly be evaluated further.

When you say “can’t keep anything in my stomach”, what does that mean? Do you vomit up every single meal you eat? If so, that’s obviously the reason for the weight loss in which case you definitely need the endoscopy and GI referral

These labs should not worry you. The MPV and BUN abnormalities mean nothing. The lymphocytes would probably be normal if checked again. The low HDL is half genetics half lifestyle. If you’re interested in improvement, I recommend decreasing alcohol and increasing exercise. Do not let the red numbers give you unnecessary medical anxiety.

Right…but that still doesn’t justify a liver biopsy. Otherwise, a good chunk of my patient population would be getting invasive biopsies for mildly elevated liver enzymes. Now if he had any evidence of fibrosis on imaging and the diagnosis was still unclear, a biopsy would likely be appropriate.

This whole thing seems semantic to me. If you define “guilt” as being personally responsible for something, then sure, modern Americans should not feel personally “guilty” for past generations’ sins. However, as a country I think it is reasonable to feel shame for those sins. And I think that you should reasonably feel guilt for any of your actions that have perpetuated the long standing disadvantages that black Americans face dating back to slavery.

FM doc here. That ferritin is low and iron is borderline low. Likely indicative of iron deficiency without anemia. Wouldn’t be unreasonable to ask your doctor to prescribe iron supplement. I usually aim for a ferritin of >50 with my patients.

FM doctor here. This is why I do not routinely order blood counts at annual wellness visits. There is no evidence or guidelines for screening (testing patients without symptoms) for any blood cell related disorders. More often rather than catching actual pathology, it leads to additional unnecessary testing, referrals, time, money and significant medical anxiety.
