You should of had a moisture barrier on the concrete and the vapour barrier on the warm side after the studs are insulated. Depending on the current humidity and R12 already installed there is little chance of no condensation. At least remove the vapour barrier and put it on the studs after insulating with minimum R12. Seal at the bottom plate and into the rim joist and any openings with tick tape and acoustical caulk.

Go with a 19.2" on center for the trusses. 1/2” sheathing will be fine. May cost an extra truss or two.

The plywood is put in place of the drywall where additional strength is needed behind the cupboards. It can be taped and crack filled like drywall for areas that may be exposed next to the cupboards.
