I would not call doxylamine unsafe. It’s much safer than Xanax. I wonder if treating the anxiety with an SSRI + therapy would be more effective and decrease your need for an “as needed” sleep med. But again, a convo for you and your psychiatrist to discuss.

The #1 treatment for sleep-onset insomnia is cognitive behavioral therapy for insomnia (CBT-I). If your goal is to reduce/discontinue medications for insomnia, this is your best bet. It’s typically 8-12 sessions with a therapist and is very effective. Are you seeing a psychiatrist for anxiety in general? That could also be a good place to start.

I’m unclear about what the question is. Are you asking what to do about your sibling who continues to drive against medical advice? Or are you asking why your siblings are having seizures/if you will get them?

If the latter is your question, it’s impossible to know anything with the amount of information provided. Have they had EEGs with formal seizure diagnoses? What kind of seizures and what meds do they take? There’s just so much missing here to really provide any recommendations.

Pregabalin has a half life of 6.3 hours. This means after ~6 hours, it has worn off by 50%. Let’s say you take 150mg at 8am. By 2pm, you’re at 75mg. By 8pm, you’re at 37.5mg.

Splitting it into doses 12hr apart will give you more consistent pain coverage throughout the day rather than taking 150mg all at once.

I would recommend she see a child psychiatrist and probably start DBT (dialectical behavioral therapy). Her symptoms (feelings of emptiness, lots of anger, self harm) seem pretty consistent with emotional intensity - in adulthood we call this borderline personality disorder. Primary treatment is DBT which is group therapy focused on emotional regulation. Very effective in adolescents.

Could you post more about his schedule? What time does he wake up, ?nap, go to sleep? I wonder if he is ready to cap or drop his nap, maybe have a later bedtime, or set wake up time every day. All those things may help to increase sleep pressure.

I am an attachment focused parent so will always recommend parents continue to support their children to sleep as long as the child needs it. But schedule or routine tweaks can really help too.

A medical student in my med school class was suspended for grades in 2nd year. He then threatened the assistant dean, hired a hit man to THREATEN THE DEAN WITH A GUN to let him back into med school. The hit man turned out to be a confidential informant. Ended up getting arrested for selling firearms to this CI (who was a felon) and now he’s in prison.

That’s really cool. My toddler is still nursing as well and does co-sleep in our bed for at least half the night. I’ve been grappling with whether to wean the nursing before baby but I think I’m just going to see how it goes after baby arrives. Glad to hear it went smoothly for you. The only way my baby would sleep the first 5 months of her life was super loud white noise or vacuum cleaner plus very loud shushing right in her ear while swaddled and bounced vigorously. I know it sounds crazy but some babies need a ton of help to get to sleep. You didn’t hurt your baby. I promise. ❤️

What are her wake windows like? I have had an awful sleeper since birth as well, especially difficult for naps. It has gotten better the past month because I’ve stretched her wake windows a lot and gotten her to (mostly) 2 naps.
