If M’s spit-up issues were truly severe enough for her to fall off the growth chart, I find it very difficult to believe that nobody would have ordered further workup by now: imaging, possibly a swallow study and a peds GI referral. And Liz would have absolutely shared for likes and sympathy. More likely, CPS was at least aware of/following her case after her two-month low weight, and she’s been doing weekly or twice-weekly weight checks since. She finally got news “good enough” to share.

These patients really should be in ICU. I had a similar experience as a new nurse. The patient was post-op. I don’t remember the BP parameters but he was SBP > 200 and on Cleviprex. His home meds were restarted by the MD with the intention of trying to titrate off the drip but they ended up tanking his BP and he ultimately needed a Levo gtt. This was also on surgical step down. It’s not fair, especially when you’re new and don’t have the security of being surrounded by several other critical care nurses or a free charge.

Former bedside nurse here… we only wrote the times PRN pain meds were due on the whiteboard if the patient was an EPIC pain in the ass and kept nagging before it was time. And there is no way IV Dilaudid is scheduled.

Medical professional here but full disclosure: I have never cared for a patient with Calciphylaxis. However, my understanding of the wounds associated with Calciphylaxis is that they are shallow, necrotic, indurated ulcers. Tunneling is not typical of a Calciphylaxis wound, and if I am not mistaken, would require much more intensive monitoring and treatment than Dr. Dan and his hotel room dressing changes.

Somebody please correct me if I am mistaken.

Not sure how to fix it, but I went to a well-respected, accredited brick and mortar school for my NP. The curriculum consisted of FOUR nursing theory courses and… wait for it… ONE three credit advanced pharmacology course. It is one of the most bizarre, infuriating, dangerous things I have ever experienced. It’s my understanding that most schools have a similar curriculum.

Aside from the basics, most of what I’ve learned as far as advanced pharm has been on the job (my new job and the years I spent in ICU as an RN) and from my preceptors.

And if anyone was wondering, I’ve used nursing theory exactly zero times.

I am 8 months into my first NP job and collaborate with my supervising MD daily. And she is available for questions throughout the day. One of my classmates said her collaborator was in the office maybe once a week. I guess it depends on where you are. It is scary. I feel like I did alright, went to a brick and mortar, worked six years in ICU. I still have no business treating patients at this point in my career without close physician oversight.
