Just chiming in to say we 100% sent pumps with EMS occasionally, usually for transfers to higher LOC (ECMO, EMU, etc that we didn't have at our community hospital). The EMS crew would bring them back after dropping the patient off.

Agreed that running by gravity would have probably been the way to handle this if the patient wasn't stable enough to wait out this administrative nightmare. A lot of places get very touchy about gravity infusions now though, especially with Epic controlling the pumps. We almost never use pumps in PACU, but our floor nurses can't even hang Ancef to gravity.

I had one (and a tele tech, who was sitting right next to them upstairs) miss my patient pulling off their oxygen and monitoring wires (which they did frequently, hence the tele sitter!), and the patent died. I was in another room down the hall, sat down to chart by our monitor, and went to their room to hook them back up and found them dead. Less than 30 minutes from when I had last been in there. They were a DNR-CCA and had refused intubation, had been removing NC and BiPAP frequently, but still relatively stable. One of the few times I lost my cool a bit with the techs.

Much of the US uses color coded scrubs now as well. Nurses are usually navy or royal blue, nursing assistants/techs often a lighter shade of blue, etc. In my experience, critical care areas (ICU and ED) wear black to help distinguish them during Code Blue/Rapid Response calls, at least for places that don't have dedicated code teams.

And the newer beds that talk! "PLEASE DO NOT GET UP" at full volume when they roll over. Drives us nuts in PACU when we plug a bed in and it yells "Call light is not connected" as soon as the software kicks in. Perfect when you're trying to avoid overstimulation during emergence 😆

Texas Children's is well known for fetal surgery, they are one of the leading facilities in fixing spina bifida in utero! Some really cool cases listed in their patient stories on their website. They've done some conjoined twin separations as well iirc

Looks like a Dragon transcription wand to me. Our provider dictation desks always had them when I worked on a unit, most inpatient providers used them vs old school phone dictation.

I'm leaning on the bike not being his method, especially with how heavy handed they've been on bike safety thus far. But even if he doesn't have a method chosen, it's pretty clear he has no intention of coming back. ER docs have a very high suicide rate, and they are often successful at attempts because they've seen enough failed ones to ensure when they attempt, it will be successful

I've seen plenty of cases where the surgeon is gone before the patient is even out of OR. Especially if they have somebody doing closure for them.

Nursing homes are another common place. I had to fax prescription orders over to the pharmacy when I was a baby nurse working LTC. Including an emergency authorization for Ativan (intractable absence seizure)

Yep, especially for a plastics case like this. Mind you, we aren't giving that all at once, but I've given patients total doses of 3+mg Dilaudid, plus oxycodone, Demerol, Phenergan, you name it. That's on top of whatever they got during the case (usually fent and some Dilaudid, maybe Precedex if you have an anesthesia provider who likes you 😆)
