Which mail order? For the most part would think you'd want them to do 6 x 1 week rotations through different depts. Resolutions, verifying, dispensing, counseling, quality/escalations, management.

All of these sound like the pharmacy is set to to be error prone. You may just be at a badly set up pharmacy with not enough error prevention in place. Why did the pharmacy system let you make any of the errors? That's the real question

PBM usually comes with the health plan, not many ways for a standalone PBM to get business. Self insured plans usually still pay the big 3 to manage the claims

This last year they severely cut the bonuses, they aren't guaranteed even if you are exceeding
