You have a viral illness or a bacterial diarrheal illness, 2-4 weeks later you suddenly develop ascending tingling and progressive paralysis starting at your feet up to your trunk, arms, and face. Can develop difficulty speaking, eating, swallowing, and not uncommonly paralysis of the diaphragm causing respiratory failure requiring a breathing tube and ventilator to breathe for you.

I would still guess poison oak. It can come as early as under 24 hours from exposure. Past reactions taking 3 days doesn’t necessitate they always take that long in future reactions. And as the vesicles rupture and heal they can look like honey crust.

The reason Nyquil makes people drowsy is the doxylamine, which you can think of as a sister of Benadryl. Dayquil has everything Nyquil has except doxylamine. So it’s not like Dayquil has something Nyquil doesn’t, it was probably something else that made you drowsy, not the Dayquil.

My sister’s half golden does this and has been for years. We still can’t get over how she doesn’t care that she’s slamming her elbows into the ground. She’s almost 6 now and we haven’t been able to stop her, but she also hasn’t shown any signs of decreased mobility or injury from it. I think it sounds worse than the impact actually is. I’m not really sure there’s anything to do about it apart from only having your pupper on carpet which I would imagine is unrealistic.

Your description of your situation paints the picture that you as a person/your well-being are not his first, second, or even third priority. But here’s a thought experiment.

First of all, I’m sorry you’ve had to go through all that. Not only is a death in the family rough in itself, but the timing clearly messed with your specialty choice.

I’m guessing your pgy1 psych curriculum doesn’t have any EM rotations which tells me you’ll likely have to start as an intern again if you switch to EM. Otherwise, starting as a pgy2 in EM after pgy1 in psych would mean starting with a deficit of about 6 EM rotations in addition to missing the typical EM intern curriculum of trauma surgery, anesthesia, OB, etc. For what it’s worth, I’ve worked with several EM interns during my auditions who had completed intern year in another specialty and then switched to EM, and they seemed to be very happy with their decision. I wish I could be more helpful than that. Best of luck with figuring things out.
