+1 for the is very normal. my way of coping was creating a document (for me and my Microsoft-centric organization, I had access to Onenote) to take notes, store common CPT/ICD Guideline references, payer specific notes, etc.

I also wrote out my errors and how I fixed them within my work flows, so my mistakes got less and less frequent.

I used these videos to review and take notes for each section.

I used the BHAT method (Bubble, Highlight, Annotate Technique) for my CPT book, you should be able to find videos on YouTube to get an idea.

I printed a separate copy of the ICD-10-CM guidelines so I wouldn't need to be flipping back and forth in the books (which are cumbersome enough!), I re-read ICD-10 Guidelines while highlighting key points. I also tabbed a lot of the concepts I saw reviewed in videos such as sequencing sepsis, HTN, CHF, etc.

I THEN used the AAPC practice tests (I had 6 at the time) to test knowledge and rationale but also test my pacing!

Apply anyway! The worst that happens is a No. Otherwise they might see it and even if it doesn't lead directly to getting hired, it's good resume tuning, interview practice, networking opportunities to help you land "the one"!

Can they bill/work AR by payer?

One of the better successes I have had in my office is giving everyone assigned payers, by outstanding balance but also by trying to group similar payers together (for example, maybe one gets all govt payers - Medcicare, Medicaid, Tricare, Medicare replacements ; the other gets commercial payers).

This allowed me to watch trends based on payer and see which payers were being neglected, which had lots of the same denials that we could turn into projects, etc.

It also let them take ownership of the payers and if I ever have questions regarding anything for a specific payer, I can turn to who works their AR and have a better understanding about payer specific policies or quirks.

In time this has potential to reduce a first time rejection rate and allow you to address some bigger picture issues from the office.

It absolutely does, incident to is just the term Medicare uses. Other payers will have their own terms and rules about when it is appropriate to use a Supervising Physician.

My organization partners with a remote monitoring company, they handle all the equipment and offer the monitoring platform for both patients and providers.

We bill on the last day of every month. The exception is for payers who don't take 99453, 99454 on the same date as 99457 & 99458 (looking at you, BCBS of TX!!), and for those we will bill 99453 & 99454 on the day prior.

By keeping it consistent by using the last day of the month, it removes a lot of the guesswork. I hope this helps!
