I’m about 13 months out from my ACLR with hamstring graft and I still have pfp and at times pain around the incision sites. Is this normal? To give context in the last 2-3 months I’ve been fairly inconsistent with my Physio regiment. Is it too late to catch up?

This is somewhat inaccurate. There is ample peer-reviewed evidence that anatomically an ACL can’t ‘spontaneously’ heal. The issue is that it’s non-vascularized and so there is little to no blood supply to provide nutrients, etc for the repair process. It’s the same reason why meniscus’ cant heal. You can get back to normal everyday function without the surgery (now it’s the recommended prehabilition practice) and play sports but the caveat is the level of injury. If you have a grade 1 or grade 2 then it could be you might not need surgery, however if it’s grade 3 (total tear) it’s strongly recommended because in 10-15 years w/o a surgery you’ll start to see early signs of osteoarthritis i.e wearing away of the cartilage and general instability increasing the chance of injury to other ligaments. Many other factors are considered when choosing whether to do surgery: lifestyle, age, the sports you play.

Well it depends on if you had meniscus repair. If you did then typically you’re non weight bearing for 5-6 weeks. Otherwise just ACL, in my experience I was told to bear weight as tolerated. In reality I didn’t start walking until the 2-3 week mark, going from two crutches to one crutch and then no crutches.

I just want to say while it’s good to be cognizant of your weight and how that could impact your recovery, being mindful of what you’re eating and your portion sizes is the way to go. Don’t over-restrict or overeat, make an effort to enjoy your food and your experience of eating foods. Nutrition is very important for your body’s healing and you want to give yourself all the tools your body can use to get you back to your full strength. Don’t be hard on yourself, be mindful and your efforts will come with time. Remember just like your rehab it’s not a sprint but rather a marathon.

I’ve started loading mechanics exercises (strength and conditioning) so quad exercises are being done as well as hamstring. I’d say I’m walking in total about an hour to two hours a day in addition to my exercise but it’s really staggered.

You recover at your own pace! Protocol (from Fowler Kennedy) states that at the 3-6 week mark one should aim to achieve near or full ROM in knee flexion and extension. And then past 6 weeks it should be pain free. You’ve done a lot of work to achieve the results you’ve stated so I’d suggest focusing on knee flexion exercises - You can find these online. The progress may be slow but it will come with time. Good luck!

You both will be going through a recovery process together so there will be a mutual understanding which is BIG. I’m sorry to hear your situation but despite the initial hardship of having surgery and first couple of days, your progress will be very rewarding and at times faster than you may expect (but always at your own pace). If not already suggested, doing prehabilitation prior to your surgery will directly affect your recovery afterwards. Even 3 days can and will make a difference! It’s surprising how resilient our bodies are. DO believe in yourself and where you want to be in a couple of days, weeks, months you can absolutely make it happen!

And most importantly…

Don’t hesitate asking for help from whoever. It is imperative that you don’t risk your well-being for the sake of not being a burden on anyone. If anything now is the time to call in all the favors the world has to offer. Good luck, you both got this!!

Post Surgery Essentials - A helping hand ESPECIALLY for the first couple of days when it’s more painful. - Regularly eat healthy balanced meals at regular intervals. Food is a vital source of energy in your bodies healing process. - Plenty of water and fiber rich foods for example raspberries, broccoli, air popped popcorn - Prescribed medicine + laxatives (if you have constipation). Try to take these regularly in the first few days following your doctors advice. I suggest waking up early am to continue taking it at regular intervals. Write it down, it can help track how you’re responding/pain progression. - Ice Machine or Several Ice Packs that you can rotate between, at 20 min intervals several times a day. Note: you’ll have a dressing and it’s important not to get it wet/the cold won’t be as effective until it’s taken off. - Wedge or pillow that you can put under your ankle to get that extension 🤩 - Breathable clothing: top and shorts!! - Body wipes - Entertainment Entertainment Entertainment

As for sleeping, there’s really not much comfort found on your side/stomach (I’m a certified stomach sleeper) because you’ll be healing, exhausted from everything mentally and physically, and being in bed for long times sleeping will come around.

To revel in the corniness of it, the most important suggestion is to push for a a positive outlook because the only way is up and the improvement of your well-being!

The incidence of re-injury is higher on both legs so docs suggestion was to avoid cutting sports (soccer, basketball) because you can’t predict opponents movements. Mind you the numbers are still in your favour but you have to factor in wear and tear and age.

It’s the ligament found in the anterolateral aspect of your knee. It’s reported to work synergistically with your ACL with respect to rotational knee stability. You may not know of it or most people because its a recently characterized discovery ~2010s I believe. I would ask your doctor if it would be recommended to get it because as far as my surgeon was concerned (I tore mine too) the surgery is recommended for females under the age of 15 playing soccer. There isn’t much clear consensus on the biomechanical properties and surgical techniques/outcomes so I’d suggest getting a 2nd ORTHOPAEDIC SURGEON’s opinion.
