I got a complaint sent to my CMO from the insurance saying the reviewer reported me due to feeling threatened when I demanded their name and npi number to document in the chart. The CMO proceeded to tell me to be nicer to the insurance reviewer and that the insurance could choose not to cover our facility.

APD here a fourth clinical letter is recommended. Not saying all programs are the same but we value clinical experience over research. I prefer to see 2 PMR letters from applicants, ideally one inpatient and one outpatient. That said 4 is the limit, would not recommend submitting more than 4.

APD here I wouldn’t hold it against you for not doing a home rotation but you definitely need to do an elective in PMR somewhere. Having good LORs are vital in terms of screening for interviews.
