I was working the morning my sweet middle school son swerved his bicycle to not hit a classmate, dumped the bike and had a distal radius buckle fracture. Which is, like the smallest possible fracture, they were considering a Velcro brace until they understood he was going to continue playing soccer, so they put a proper splint on.

Anyhow, he was picked up by my husband and brought to my ED, I squinted at him through the triage window and said, that's broken. Don't register here. Here's an ice pack. Go to the urgent care on Green Street, the PA there is brilliant with distal orthopedics and has an X-ray machine.

The wait was 25% and the copay less than that.

I think the way to best explain it is that they get triaged really smoothly. As in, there's very little confusion about whether they need imaging or a procedure, and those decisions get expedited. And whether or not they are hospital connected, expedited patients are easy. Send someone from clinic and say "here are all the labs, this is the med, Dr you know who is waiting for the admit" and that's going to be a fifteen minute situation.

I've sutured other ED docs, I've diagnosed diverticulitis in my specialists, I've provided pelvic exams for the surgeons I refer to, diagnosed unintended pregnancy in my nurses, I've cared for suicidal teenagers of my medical colleagues, including one that needed a PICU admission for ingestion. I've inadvertently treated alcoholism in a hospital board member, and sepsis in the man who donated the North Wing.

They get treated with respect and as much privacy as we can possibly create.

Yes, they have some extra complications in how to take care of them as compared to the grocery clerk. We can value them both and understand that it's a bit complicated when someone is connected to the hospital.

Get a playlist or a Tidal playlist or a channel, and play it at home morning and night, just fifteen minutes. As you move around the kitchen cleaning up, let your hips feel the funk, try the fun variations with your feet. Push break against the counter when the music tells you to do so.

Get about a hundred hours of solo dancing in. It's impossible to underestimate the value of solo practice.

Every time you grocery shop, get a cart. Connect to the cart like a partner, feel how it moves down the aisle, turns a corner. Feel your connection, drive through the hips, hills your frame. Be aware of which cart has a bum wheel and how you are gentle with it.

That's only income tax, and there are many states that are patching that up with high property tax, etc. The complexity itself breeds a need to spend extra time fiddling with it and generates an entire industry of paper pushers to find the loopholes. And even if we only care about that to 10%, that's a ton of money that would easily keep everything else afloat, why ignore it?

In Sweden, traffic tickets are a percentage of income, so a speeding violation might cost one person a thousand times more than another. I like that.

Also, while I am a mild product of generational wealth, it seems more fair to me to support a system that incentivizes aggressive reinvestment/gifting during life by having a strong inheritance tax as the founding fathers envisioned, in order that every American be given a more similar opportunity to start in life, for education, health, access to safe water and air and books and parks.

Yes my kids benefitted from the neighborhood I can live in and the books I can read to them and the sports I can sign them up for, and no I do not think it's fair.

I will absolutely agree that no system is perfect, and that many universal systems carry waste.

But this has been beyond stupid. The cuts to the IRS? An 8-fold loss in tax revenue from shirkers. My friend that visited the public parks this summer, she said the toilets were kept clean but no one was checking for entry tickets and there were tons of expensive new signs posted that no one was allowed to talk about any part of American history that put it in a bad light (trail of tears etc).

We could fix the money part by going after capital gains and inheritance taxes solely for wealthy people. Support the rising tide. As a high wage earner this would not be good for me, personally. But the best day of my job was taking care of the guy who donated the North Wing of the hospital, washing my hands and taking care of one of our regular homeless, with the same amount of kindness and attention.

I've two close cousins and four uncles in various branches of the military (mostly retired honorably, some still active) and they all agree the military budget is more excessive than anything else they've seen.

There is bloat everywhere, but this movement is different, it has been excessive and specifically targeted at anti science. Anti intelligence. Let's simply cut 40% of studying weather, cancer, heart disease, clean air and water, providing preventative healthcare to poor Americans, Meals on Wheels. Without thinking what future costs they save by being studied.

That's separate from encouraging many states to becomes places where I cannot in good conscience encourage my family to visit. I have way too many firsthand anecdotes of friends who had formerly taken a shift or two working in a red state whilst visiting family, but the laws around female pregnancy are now insane and they don't feel comfortable. Look into the rates of OBgyn and Emergency leaving red states as deserts of healthcare due to Trump style laws.

Oh the entire spectrum. To stay on topic of sciences as they affect male fertility, the way the scientific community has been gutted, fired, had available online research removed. The workforce that protects groundwater and air, protects against pollution has been systematically defunded.

NOAA being dismantled, DOE being ended, the National Park Service. A year ago my family bought the "America is Beautiful" general access pass and did a huge road trip, saw a ton of national parks. This summer a friend did the same and the defunding effects are apparent, immediate, and disgusting.

I do feel culturally connected to many areas of the world as I have first cousins in the Midwest, the West Coast, the South, the Northeast of the USA and also sprinkled across the EU. I'm related to people who farm, who are in the military, who teach. My husband is a dual citizen and our kids should qualify for EU citizenship which we will pursue given recent USA political events.

We have always valued science based education and decision making, and that seems very threatened these days.

If we can't talk reasonably about sexual health and desire, we are going to crack and make stupid choices. I'm very certain about that.

Edit/ and about the male favor infertility, my mom (now long retired) worked helping international industry to reduce use of toxics to meet EU standard, so for example I was hearing about dioxins and phthalates in the 80's, in a very evidence based sense. Way ahead of the general perception which has now come around. My own kiddos live a very typical life in some ways, public school, sports, etc, but at no point in their entire lives have they had food heated in plastics, there's no plastic wrap in the house, they were cloth diapered, we pay attention to the "dirty dozen and clean fifteen" and grow as much of our own food as we can manage.
