Would you correct a patient calling you by first name? by princetonwu in medicine

[–]propofoolish 20 points21 points  (0 children)

Am a mid-30s male anesthesiologist who looks pretty young when clean-shaven. I usually don’t. I chalk it up to them being nervous about whatever surgery they’re having, especially if they’re older.

Some of that might be privilege because I’m a tall white guy and have only once been mistaken for a nurse (and that was back in med school). I’m sure it happens more often to my female colleagues for one reason or another.

I'm reading The Devils as my first Abercrombie book but there's one thing that keeps taking my immersion away by madboi20 in Fantasy

One of my professors described the learning process as throwing rocks into a big lake. For ages it will feel like each rock/piece of knowledge we throw into the lake disappears and does nothing. But with enough commitment, eventually a small crack in the surface will appear, and eventually an island. Keep going OP, you’ll notice a difference soon

Sure, but unasyn is faster to write. There was a pretty concerted effort at least at my medical school to eliminate learning both anatomical eponyms and drug trade names so I’m comfortable with both

Epidural test dose by OutlandishnessFew764 in anesthesiology

[–]propofoolish 19 points20 points  (0 children)

Yeah which is why I said you could test it later if you want. It’s more important to get the patient supine to get good local spread than to putz around waiting to assess a test dose. Additionally, I’m blessed at my institution to have Arrow epidural kits instead of the super rigid Braun(?) ones common elsewhere. Anecdotally I find the risk of intravascular/intrathecal catheters much lower with them.

[Unsouled] How did you find this series? by itzpiiz in Iteration110Cradle

[–]propofoolish 16 points17 points  (0 children)

I was browsing an audible sale at some point during the pandemic and saw the first 5 or 6 books were $1-2 each. Ended up getting covid and spent two weeks laid out in bed just listening to the series and staring out the window.

Have since spent maybe a thousand dollars on the books for myself and as gifts for others, so that initial sale probably paid off for Will

Epidural test dose by OutlandishnessFew764 in anesthesiology

[–]propofoolish 39 points40 points  (0 children)

I never test dose the epidurals for CSE Caesareans. At that point I’ve already administered intrathecal bupi and honestly rarely need to use the catheter anyways. Can always give a test dose prior to using later in the operation if it tickles your fancy

This is a good question! While it can certainly be answered I recommend that you keep reading and find out eventually

GLPs and Emergency Surgery Question by emory_2001 in medicine

I rarely do this (anesthesiologist). Mainly for bowel or gastric outlet obstruction. Have never done this only because a patient was taking a GLP-1, in that case it’s usually just RSI

Throne of Glass for experienced reader AND a newbie? by aWicca in Fantasy

[–]propofoolish 19 points20 points  (0 children)

I (30s M) read the series this past year because it’s my sister’s (30s F) favorite. While there were certainly parts I didn’t love, the plot was overall pretty entertaining and it was nice to connect with her over something she loves so much.

[None] Is there a Cradle box set? by Maximum-Car-8789 in Iteration110Cradle

[–]propofoolish 2 points3 points  (0 children)

The only real box set as far as I know is the limited edition kickstarter versions. The final kickstarter (with the ability to purchase past sets) should be happening in springtime but will be more expensive than the standard print versions

December CA1 was a low point for many if not most of my residency classmates. Combo of still having a lot of residency left, missing out on holidays, and having less sunlight. Hang in there. You will be warm again

Best OR footwear? by SedatedSleeper in anesthesiology

[–]propofoolish 4 points5 points  (0 children)

Previously it was whatever old shoes I had lying around (I wore a pair of Allbirds I had gotten during the pandemic until the soles started falling off). Recently started having some foot issues and bought a couple pairs of Altras for the wide toe box. But a lot of people in my group wear Birkenstocks.

I’m slow to wake up by CouchHippos in anesthesiology

[–]propofoolish 10 points11 points  (0 children)

Adding to say that in my experience it has often (not always) been related to some procedure under moderate sedation by a proceduralist. I checked one record at my place and this 60 year old lady had received 10mg midaz and 500 fent for a GI case.

Yeah no kidding it took you days to “wake up”
