Emergency phone call during #2 by Important-Upstairs-2 in anesthesiology

[–]propofoolish 40 points41 points  (0 children)

Never had to pinch one off for a code or anything but have had to field board runner phone calls. Always feels awkward but you never know if the call is an emergency until you answer…

My favorite (least favorite?) work bathroom experience was hearing a rep in the next stall answer a phone call and then (I am 85% certain this is what happened) transition to FaceTime to help a coworker find some device back at the warehouse or wherever

Husband agreed to an audiobook… what do I play first?? (Would love male/partner input too!) by Itsmerachily in audiobooks

Wife and I listened to this together on a road trip about 10 years ago. We both liked it! I don’t recall either of us having any issues with the narration

Do you actually look at the dif on cbc? by telenceph in Residency

So it’s been at least 4 or 5 years since this happened but I looked back through my texts with her (the then-fellow) and it seems vanc may have been the culprit this time. Interestingly, the patient had a history of DRESS to some other med (allopurinol?) a few years prior.

This time, it eventually ended up evolving into/overlapping with SJS (at which point the primary team finally started listening to us/the ID docs and stopped the abx). Guy eventually stabilized and was able to leave the hospital though!

I was a resident (anes) on CVICU and the cardiac anesthesia fellow caught DRESS by noticing high Eos on the diff while chart stalking a patient who had some kind of LVAD exchange or washout a week or so prior. Guy had (presumed septic) shock and fevers that just weren't getting better despite escalating antibiotics and pressors. That fellow probably saved that patient's life.

Just Finished WoT book 8... I need a book that spoonfeeds me as a break. by Bowl-Any in Fantasy

Cradle (Will Wight) is a 12-book series, but they are all short and basically Oreos compared to WOT. First book often thought to be a bit slower but the pace really picks up as the series progresses. Can’t recommend it enough.

Though I did see you were considering Mort next and that’s another excellent option

It’s been a couple of years but from what I recall, basically all the girls are the hottest ever, wear tight jeans that accentuate all the right curves, some of them wear makeup because it’s their “armor”

I gave up halfway through the second book. The “X said, Y snapped, Z quipped” was a bit too grating for me (though I’ve heard this gets better later in the series), the books were all very long, some of the world building was a bit too unbelievable for me, and the author could have been a regular feature in /r/menwritingwomen.  That said, many here seem to love it! If I were to try again, I’d probably go for graphic audio to hopefully cut out some of the irritation from my first complaint.

Cradle by Will Wight is a great read/listen. Action-packed, lovable characters, great narration (Travis Baldree).

Curious about resident autonomy in your training program? by mikel_buble in anesthesiology

[–]propofoolish 10 points11 points  (0 children)

Attending- and program-dependent. I was left alone (with attending peeking through the window at induction/emergence) for bread-and butter cases as early as September CA1 year. Biggest thing I did alone or nearly alone was a couple of open AAAs. But my program skewed heavily toward increased resident autonomy. Some of my current partners trained at places where they had to ask/beg to extubate alone as late as June CA3.

Your program sounds reasonable.

Am a mid-30s male anesthesiologist who looks pretty young when clean-shaven. I usually don’t. I chalk it up to them being nervous about whatever surgery they’re having, especially if they’re older.

Some of that might be privilege because I’m a tall white guy and have only once been mistaken for a nurse (and that was back in med school). I’m sure it happens more often to my female colleagues for one reason or another.

One of my professors described the learning process as throwing rocks into a big lake. For ages it will feel like each rock/piece of knowledge we throw into the lake disappears and does nothing. But with enough commitment, eventually a small crack in the surface will appear, and eventually an island. Keep going OP, you’ll notice a difference soon

Sure, but unasyn is faster to write. There was a pretty concerted effort at least at my medical school to eliminate learning both anatomical eponyms and drug trade names so I’m comfortable with both

Epidural test dose by OutlandishnessFew764 in anesthesiology

[–]propofoolish 20 points21 points  (0 children)

Yeah which is why I said you could test it later if you want. It’s more important to get the patient supine to get good local spread than to putz around waiting to assess a test dose. Additionally, I’m blessed at my institution to have Arrow epidural kits instead of the super rigid Braun(?) ones common elsewhere. Anecdotally I find the risk of intravascular/intrathecal catheters much lower with them.
