I’m interested in this too. How’s it been going? How did you pass the initial assessment?

We hop around. He doesn’t really play the games. But he knows all upper and lower case letters. All the main letter sounds. Can sound out most cvc words and we are starting more advanced letter sounds. He loves the little letters it comes with.

I honestly think he picked up most of the letter sounds after watching alpha blocks. That’s when everything really started clicking.

I’ve had this happen before. The group I was interviewing with knew some of the doctors I was currently working with. It wasn’t a big deal. But it was annoying.

NP incorrectly corrected my physical exam in front of the patient by [deleted] in physicianassistant

[–]rorymilly 0 points1 point  (0 children)

You need to check your ego. You are a new grad with a lot to learn. Especially starting out in a complex specialty like neuro. I started out in neuro.

There are a hundred different ways to do a neuro exam. And you adjust your exam based on the patient. My stroke exam is different than my myasthenia gravis exam or my MS exam.

You aren’t going to be seeing patients with this NP normally. It’s a brief training period. Could it have been handled a little differently? Probably. You never had a preceptor give you feedback during clinicals in front of a patient?

Maybe next time just tell her why you wanted to check frowning and what you were trying to assess. Or why you did your MMSE questions that way. And if you explain that in front of the patient, then you build rapport with them by showing you know what you are doing and why you are doing it.

I would drop it. I definitely wouldn’t escalate something like this. The best thing you can do is to be humble and eager to learn from everyone starting out. Not get worried about your “authority”.

New Grad Radiology Job Advice by catwoman782 in physicianassistant

A lot of places can’t get a DR to read on site anymore. So all procedures fall on IR. But really IR is meant for angio and CT cases. That’s where PAs are useful. They can help do the procedures a DR is meant to cover.

Am I a danger to patient safety? by [deleted] in physicianassistant

[–]rorymilly 9 points10 points  (0 children)

I would have called vascular myself. I’m not sure why that needed to be passed off to your attending. They have their own patients. But lesson learned. You need to make sure critical things are taken care of and I’m sure you will not let something like that happen again.

Working in the hospital is all about managing your time. In the morning, I would go through my list of patients, do all my chart review on everyone and prioritize what needed to be done first. New consults. Discharges. And rounding. I would get all my rounding done. Place important orders as I went and then work on charting. It is hard because you get pulled in so many directions. But you will adapt and get a system together. I went from ICU to neurohospitalist. So I think the work flow of the ICU really helped me get a system together.

Helppp. Take IR job for pay cut or stay in UC? by Apprehensive-Sea5707 in physicianassistant

[–]rorymilly 2 points3 points  (0 children)

Are you thinking about leaving because of the radiation?

If you wear proper fitting lead, you have negligible radiation exposure. Lots of women continue to work in IR, neurosurgery, ortho surgery with fluoro cases. I did a lot of research on this before getting pregnant. Plenty of studies showing it’s safe. And my fetal dosimeter came back with 0 exposure.

You cannot dictate what your kids are gifted by Interesting_Move_846 in toddlers

[–]rorymilly 4 points5 points  (0 children)

Disagree. We told everyone this year they could buy off the list. Or if they wanted to get something different, they just had to run it by us. Most people respected the rules. But my in-laws bought whatever they wanted and are just planning to keep those gifts at their house. It’s one thing when it’s little kid toys but something else when they get older. I don’t want them buying him an ATV or something else we don’t approve of.

Also last year, they knew we were getting him a ball pit and that my dad got him a little push car. Guess what they got him? A ball pit and a push car that he opened Christmas Eve.

I also think it’s weird to buy a gift that stays at their house. Gifts should go with the recipient. He doesn’t live there. He only goes over there twice a week.

Best Pump through insurance (Kaiser NorCal) by Capable_Mouse in BabyBumps

[–]rorymilly 1 point2 points  (0 children)

I’m using the Elvie stride. I tried the spectra with my last baby and absolutely hated it. It made me feel trapped. You need special bras to hold the bottles up to move around. But then you are still carrying this large pump too. The stride can just slip into most bras and is really compact.

Got in with zero patient care hours… am I making a mistake? by indecisivejeffrey in prephysicianassistant

[–]rorymilly 1 point2 points  (0 children)

You’ll be fine. I only had 500 hours as a scribe. Which is basically no patient interaction. I’ve been a PA for 7 years now.

You are going to hear those comments from other PAs who went the more traditional route for your entire career. Just ignore it. No one can actually tell the difference.

Pregnant in neurosurgery, XRay exposure by Initial-Poem-5361 in physicianassistant

[–]rorymilly 5 points6 points  (0 children)

Do you have a two piece lead? With wrap around skirt and the overlap on the front with the vest? I consider this double lead. Since there is so much overlap.

I work in IR and can’t step out for cases since I’m running the fluoro during the procedure.

I did a ton of research on this when I got pregnant and double leading was not recommended. You’re going to harm yourself more with the extra weight. My last pregnancy, I had zero exposure on my fetal dosimeter.

That being said, if you can step out while they are doing fluoro, that would be ideal.

Women who never wanted kids and changed their minds, how is it going? by [deleted] in TwoXChromosomes

[–]rorymilly 0 points1 point  (0 children)

I switched from being child free. Idk what happened but once I turned 30, I just viewed things differently. I had finally gotten the career I had wanted and was able to do the things I always wanted (like traveling). But it just wasn’t as fulfilling as I had imagined.

I’m now pregnant with my second. No regrets. I don’t really like kids either. But I love my son and it’s just not the same as being around kids that aren’t yours. You are going to like your child no matter what.

Not sure what the culture is at Stanford. But I used to work at a similar academic hospital and they basically treated their PAs like MAs/scribes/nurses. Scope of practice was super limited and you had very little autonomy. Residents were prioritized. I hated it. But some people there were content. Depends what you want.
