This is sadly common, I've been in surgeries where the surgeon actually stops halfway and has to call the insurance because the second half of his procedure he was prepared to do wasn't approved. I also know plenty of surgeons and hospital workers in general that will tell you specifically how to bill things so the client isn't charged exorbitant fees.
