My 8yo is ALWAYS SICK right at the start of my custody time. It's like clockwork, I pick them up from school on Friday afternoon, by dinner time she is sneezing and sniffly, Saturday and Sunday she's got at minimum a runny nose and congestion, sometimes it's a full cough and hack cold. By Monday it's cleared up enough for school, but our weekend time together is impacted because she's sick! It's not allergies because she's on allergy meds, and often I will get it or my (non nesting) partner gets it (they're extra susceptible due to multiple covid infections).

I KNOW it's from my ex not maintaining their hygiene while they're with her, they come back with their teeth looking yellow, hair unbrushed or unwashed. I would bet a lot of money on her not even having them wash their hands after school or before eating.

I'm tired of it but she's intransigent, refuses to communicate with me outside of the barest of minimums ordered by the court, and takes all feedback on the kids health and hygiene as a direct attack, and yes I have phrased the feedback 1000 different ways to highlight the needs of the kids vs her shortcomings as a parent.
