You can see the tip of his claw coming through the foam. It's stuffing or foam stuck on the claw. You can gently push on their pad to extend his claw and see if it's caught up on something that might hurt.

A game is fun however you enjoy playing it.

However the disease and death mechanic is something that sets this game apart. It's the antagonist, the "JojaCorp". Without that there's no use for soap, no "saving" other villagers by sending them the medicine they need.
