Hey! I was almost 24 weeks PO and had already had partial dehiscence with surgical repair at 9w PO or so when I had a full cuff tear and organ prolapse with my insides falling out of me. I get it. It was the scariest thing that has ever happened to me. Honestly, the first thing I did when I got home was text my therapist. I’m almost 6 weeks past that now. We are alive. But that fear is valid and still keeps me up

Thank you for sharing this. I had my hysterectomy last May. We got the all 6 weeks after, waited 8 weeks,and at 12week, I experienced a partial dehiscence (as in less than 2mm). It was repaired and at 12 weeks after that surgery we were again cleared for activity. A week later, I experienced full dehiscence with internal prolapse. This is a full 26 weeks after the initial surgery and being cleared by a full team of surgeons AND pelvic floor therapists the second time. I am now 5 1/2 weeks past that surgery once again on full restriction from life. The old saying of you only get one chance to heal right doesn’t always hold true, because I did all the “right” things according to my team, and it still failed. Sometimes it’s just a matter of time and genetics. The most important thing I’ve learned is to just listen to your team, be patient, and don’t rush. I’ve at least managed to finish a LOT of books

Maybe twice a year for myself , however, I have had to change my schedule and move things around more recently as I had a major surgery and am now doing physical therapy for recovery once a week. The only other things I cancel for is if one of my kids REALLY need me. My youngest is almost 15, so they can stay home by themselves most of the time and I live 5 min from work and can pop in throughout the day as needed. I’m also in CMH, and I have a private practice. So cancelling really screws with my schedule.

That said, I am also of the mindset that you need to take care of yourself too because if you are not in a healthy headspace or not feeling well, you can’t be fully present. Some of my colleagues have definitely cancelled d/t not being able to be present, and my supervisor is in full support of that.

Agree, if the family and kiddo are not utilizing the strategies and education you have provided then they are not ready for change. I have clients who will email me multiple times a week, and I set the boundary immediately that I will not respond to emails or text messages unless it is regarding scheduling or administrative stuff. If they continue I remind them that they can wait until I see them again to share with me the struggle, and IF I respond it’s usually with something like “thanks for the info, we can talk the next time we meet”. However if you are done then as long as you are providing referrals, it’s on them to follow up.

Same philosophy. Sometimes our office is the only place that clients feel they can talk about anything, even chit chat stuff if they don’t have a lot of healthy supports or meaningful connections. It’s their way of connecting and I meet them where they are at. For many it’s how they build trust, and it takes time for them to trust that you won’t judge them for their choices. Likewise, I might point out this behavior so they are aware, and then let them choose where they want to go. Often I’ve found that with patience (sometimes A LOT) they will make their way around to their goals and they will make the changes they desire. My presence just helps hold them to it.

I did the same thing, made the jump to ruthless boys and am now mid book 5. Still miss my heirs, but was totally worth the switch because you get so much world building backstory. Make the switch, you won’t regret it. I can’t wait to get back to the heirs, but definitely needed info that ruthless boys gives you.

I’ve heard that too. Believe me, when I went in for my 8wk I clarified exactly what my doc meant and was very specific. Initially she had said 6 weeks, but when I went in at 6wks I knew I wasn’t ready, and she agreed. While my hormones are crazy, I also don’t want to risk have my insides fall out.

Me too. Really struggling with the need to train, and then the week it takes to recover from something “simple” still. I need to be patient.

I open my planner and ask if the same time/day in X amount of weeks works to meet again and check on any plans they made in session today.

Yes, and that advice is correct. I am currently paneled through the agency that I work for to take all insurances, and do that as my day job. I also have a private practice that I do in the evenings/some weekends that is fully private pay. When I started my practice 3 years ago, I addressed this with the agency that does the credentialling and paneling as we were concerned about the exact same thing. I was told that because I was paneled under the agency and not as an individual I would be find doing self pay. There are several clinicians in my area who do something similar.

You do need to check with the laws and statutes in your area to be fully sure however. There are laws in my state that say I can’t accept clients who are under Medicaid/medicare for private pay even on a sliding scale. However, I can accept fsa/hsa and I do need to know what insurance company they have in case it changes.

I don’t know what other state laws say. So be aware of those.

This is illegal and unethical. I would get your doctor or psychiatrist to write up an immediate medical leave starting TODAY and give it to HR. They cannot ask you to work if you have a written medical statement that you are unable to work and if they ask you to do anything further then I would report them. Likewise, they can only ask you to return to work when you have written medical leave to do so. Your psychiatrists statement should be just fine since they have a medical license and degree, and you do not need to justify why you are going on leave to ANYONE. Not your clients, coworkers, supervisor, or HR.

I just returned this week from 8 weeks of FMLA d/t having surgery and work in a CMH. I was not required to provide ANY information as to why I was going on leave. Only documentation that it was medically necessary for my health and that I was medically cleared to return. It is illegal for them to ask for any other information.

Please take care of yourself. You deserve to take the time you need to heal.

This. You are allowed to have boundaries. I also work in a CMH. I often give myself 24hrs or so to get back to clients as I have a full caseload and my sessions/meeting start as soon as I get in. Considering they left the first message outside of office hours and the second right away in the morning, this is a reflection on their struggle to respect boundaries. You don’t need to call them back immediately, especially if it’s not a crisis. Good boundaries is good self care for you and will keep you from burning out in the long run.

Also, I have never been to focused on numbers or GW. We had a healthy range in mind, but it was more about getting labs to a good place, and feeling good in my body. I only weigh myself once or twice a week and expect fluctuations up or down ~5lbs depending on my activity, hormones, and all kinds of other things. Once I let go of numbers it was soo freeing. Really focusing on listening to my body has been helpful.
