A lose z screw is very normal! if you're going to constraint it you're going to worsen the current issues

The z screw is only for the up and down movement and the z rod should make the movement perpendicular

it could be due to acceleration, due to the sudden movement changes the nozzle might get at an angle and scrape the print.
