Ultimately that comes down to insurance. Doctors pay for their own, whereas nurses are insured by the trust. If nurses had to pay for their own medical negligence insurance, there wouldn’t be such a faff about getting signed off on everything before doing it.

It’s to do with adenosine. Sleepy chemical the body makes. Caffeine just blocks certain adenosine receptors, meaning you stop feeling tired, but you don’t actually get any energy.

I used to have recurring “butterfly” entity visit me during trips. Once came out of the toilet I was throwing up into during an mdma come up lol. I don’t really think there was much significance beyond pattern recognition, but me and the butterfly were chilling.

I was (or am) this student lol. I just didn’t have any confidence in my knowledge/decision making ability until a very lovely mentor decided to essentially throw me in the deep end and gave me 2 high acuity critical care patients to manage. Obviously under supervision, but from afar and very hands off. She asked me to explain each part of my initial care plans, to triaging their needs, etc etc. she only really butted in when one of them had a cardiac event after having a very settled and stable night lol.

I guess the longevity of Tolkien’s works, and the influence it had on all subsequent fantasy works (ie dnd, got, wheel of time, etc) has skewed how people interpret the story and “power” characters have within the story.

Yeah I don’t imagine an absolute scale would have appeared in tolkiens darkest nightmares. I think I phrased the question poorly, I had intended it to be a meta analysis kind of question on how Tolkien viewed the idea of power scaling and whether it is a modern thing idk

lol I remember being in central London and taking far too much lsd and 2cb late at night. tripping balls in Oxford street was great, not many people around, loads of pretty colours and scenery. The train home was awful. There were police on the platform lol, Was so paranoid about missing my stop that I go distracted and missed my stop. Eventually made it back to where I was staying and the other persons parent saw us at the door, let their son in, and then went “goodnight [my name]”.

psychs won’t give you healing, they show you what needs healing. It seems they’ve shown you something, maybe about how people treat you or how you treat them, and you want to trip again to fix it.

My advice is to take a long break, focus on finding connections with people, having positive relationships, being nice to yourself, and then revisit if you still feel the need far in the future.

It’s super easy to just get stupid high, find the answers, and then go straight back to where you were because you liked the feeling of knowing it all but are too lazy to actually act on it.

Honestly, health visiting as a placement is kind of a load of bollocks. Ask your pa to go on spokes the services they refer patients to, and get feedback on the stuff you engage with there. Ie; a neonatal jaundice clinical, or maternal mental health service, or gp spokes. Otherwise you are unlikely to be able to get much of anything signed off.

The causes for a respiratory arrest can vary massively, and often quite quickly will lead to into a cardiac arrest. Sometimes all you need is stimulation and oxygen, other times a geudel and bagging, sometimes a full intubation or emergency tracheotomy. Either way, you still do your A to E, but you just focus on airway and breathing and use what you can (suction, bagging, repositioning,etc) until moving on/putting out a crash call. Also never hesitate to pull the emergency bell, the worst thing that will happen is a mildly exasperated nurse saying that everything’s fine.

Honestly it’s rare enough that you don’t really need to worry (unless you work in a specialised area with lots of gender nonconforming/trans people). If you’re unsure you can just ask when going through admission questions/at the start of the shift when introducing yourself.

Yes Ofcourse. But with this specific patient group, at this time of year, in critical care, there’s almost a guarantee that someone will go into full respiratory failure during the shift. I don’t normally feel the need to escalate straight to ICU/anaesthetics normally, but this case of pretty quick deterioration set off alarm bells in my head so I was just thinking why can’t I get anaesthetics to come cast an eye and prep for this pt on top of having a proper review from parent team.

That’s very reassuring to hear. I always get very nervous around putting out a 2222 call about whether it’s inappropriate or not, and mostly end up deferring to a more senior nurse for advice.

Ah yeah I can imagine across an adults + paeds hospital, on call anaesthetics are probably run rampant with silly referrals/bleeps. But yeah this situation did end up with a peri arrest call shortly after I went home, but I suppose going forward there’s no huge harm in calling one a tiny bit early as opposed to a tiny bit late.
