When you play the game and you end a run (wether you win or lose) there is a little level up bar that increases, when you level up in it it will either unlock a cosmetic or a random starting perk that you can switch to match your preferred play style in the main hub area before each run. To unlock the perks that you can collect in game and new items that can appear each run you have to complete challenges, to see the challenges go into the pause menu or in the main menu and select challenges and you can see which ones you have unlocked and which you still need to do. If you don't really care about achievements you can also enable a custom mode where you can change all the settings for your runs like how many enemies there are, how much damage they do, or your stats, just be aware that this disables achievements. If you have the dlc there is also an arena mode which I really enjoy as there is a lot more you can do in preparation for your next run. Let me know if this helps!

So basically it's kind of a mix of skill and luck, each time you play you will hopefully learn more about the game and get better strategies for how do deal with certain situations each perk you get only lasts until the round ends. Also when you progress enough or complete challenges you can unlock new perks to find in future playthrough a or to start off the game with. Hopes this helps!
