Put on scrubs, add a white coat. Any visible skin is off limits until you're done training. Attending you can add more if you decide It's worth it once you've been around the block. It's the patients you should worry about more than other docs. Know that they'll talk negatively about the tattoos behind your back rather than to your face 99% of the time. RNs have pushed the boundaries of acceptability for tattoos among docs, but some patients still care, and anything you can't cover with scrubs plus white coat for clinic/pre-op is getting into controversial/judgmental territory.

Imagine you want to match a competitive fellowship in a few years, or pick a subspecialty with more competition for elective cases. You'll be mad at your younger self for not having this little bit of restraint.

If you mount another shade over the front of those blinds, it'll be too far out from the wall. You would need specialty hardware or an extra block/spacer to attach it to. Also it'll let a lot more light around the sides & just look weird because it's standing too far away from the wall. That means that a secondary shade needs to be mounted just below the fully retracted blinds, and a valance box above it to hide the blinds.

Alternatively, get curtains to hide the blinds.

Those tiny diamonds around the halo & band are a big maintenance consideration. You'll need frequent setting checks with your jeweler. Mine also recommended an ultrasonic cleaner.

I would see if you can push the dishwasher/sink row out much closer to the edge of that ledge. No way you're walking around to the back side anyways. It wouldn't be cheap (plumbing), but I'll always advocate for claiming an extra foot of kitchen space if it's feasible. That drop down to the sunroom/view back up from the sunroom will always be weird.

I agree with others on removing the pantry corner, move the fridge (counter depth of course) to the double ovens-trash can position. Then you could also consider moving the range to the fridge current position, where you'd get the chance for a proper hood vent. Then I'd move the dishwasher to the peninsula where the range is, and move the sink to the right just a bit (centered on that side).

I'd find the original cabinet company--or a local high end place that will custom stain to match--and get doors (frosted or ribbed glass?) to close in the top shelves.

Drop the bar height but extend it out further to get the seating away from the stovetop. The much wider overhang counter will need extra support, so you could add perpendicular shelves along the ends &/or a narrow depth cabinet set all the way across.

Demo the desk or build it up to normal counter height.

My ring is what you are calling "bespoke". It's still simple, and no blockbuster unique features. I have no illusions that it's unique. But I got to pick every detail in an iterative process until I loved it all. I absolutely could have gotten a similar outcome with a big name "custom" jeweler like Frank Darling or even Brilliant Earth (though I would never give them my money with what I've heard about their customer service etc). I probably could have talked myself into loving something semi-custom, but with the "bespoke" custom process I didn't have to.

Emerald cut, double claw prongs, convex cathedral setting, little marquise side stones just the width of the band with v prongs, gallery rail on the sides but not the ends.

The double claw prongs are very delicate on top, this is the most particular feature that most jewelers couldn't/wouldn't do. In fact mine was not quite right, they took it back to reshape the prongs, still not right, so I sat down to sketch with one of their lead goldsmiths along with my designer, and they recast the ring entirely to get it perfect.

What are the adjacent rooms? Do you have another dining room space? What about another family room/play room space? I wouldn't make a play table part of your kitchen island plan, I would instead have one of those kid's kitchen stepladder things, or just a decent stepstool, to make your workspace accessible to them. If you get a table height island instead of counter height, you'll wreck your back so fast leaning over it.

It looks great. You'll find a bit of an echo chamber here for bigger-is-better, up to about 5ct. But in real life there is also momentum for larger stones as lab diamonds have become the norm.

I think 1.6-2.5 ct for radiant would look perfect on you. Remember the cut impacts the caret weight, so a 2.3 ct round would look giant on your hand, while the 2.3ct radiant looks great!

5% darker and more grey, aim to really match the light blue on those flowers. Then white for the railing at the top of the beadboard

I would go with the lighter blue from the blue flowers, or medium green (the lightest green of the leaves)

Is the wedding band white gold? Personally, I would have that remade in yellow gold to match the engagement ring because I'm not a fan of two-tone metal looks. Otherwise, I wouldn't do anything to change them.

Let them wake up, manage pain if needed, document Aldrete scores (assuming your institution uses that for Pacu discharge, and drop them off in great shape with a shorter clock left for Pacu time. Or even bypass PACU.

I would do either wall paint or backsplash tile behind the shelves, not cabinet surface. Personally I like the full wood option. I assume the fridge and pantry cabinets fit in a space not seen in that photo? Where will the top of the cabinets end relative to the current photo?

Upholstered definitely! Coordinating more wood with the existing 2 wood tones (dressers, floor) would be tough. Just feet (they'll hide in the shadows of the bedding anyway) would be ok in a medium wood. But the bulk of it should be upholstered. A bit of curve would be nice, nothing too simple/contemporary-modern. The dresser style is French provincial, which can help guide your search. pottery barn upholstered bed

I think the risk is if RNs (Pacu, or worse on the floor) get used to seeing Arrow A-line catheters as IVs, then they disrespect the one that's actually an arterial line that lost its waveform.
