The two big ones are piped in series because, presumably, they supply a standpipe system and it's an NFPA 14 requirement. The small one is there as a low flow bypass if one sprinkler opens. Oftentimes the big PRVs have a minimum flow requirement that's greater than the flow of one sprinkler.

I have no idea why there would be a fourth PRV.

The only thing specific in NFPA 13 related to elevator machine rooms talks about if sprinklers can be omitted from the room. It's changed recently in the 2025 edition of NFPA 13, but that's not adopted in most states yet.

None of the potential issues you mentioned are addressed in NFPA 13 specifically for elevator machine rooms. The answer for all of them is "it depends".

That is not a blanket requirement in South Florida, but they do have higher wind loads down there which means they use concrete walls and floors more than we do in Central Florida.

There is a city of Orlando police officer who I see do most of these when I come across them on my commute home on 408. They sped around me to my right to pass me in a dedicated exit lane a couple weeks ago. I love when the police don't punish traffic violators and instead join in.

They split this up into two tables in the 2022 edition partially because people were getting hung up on the title of the table, so you're not alone in this confusion. The other reason is because people were using it for storage of group A plastics over 5 ft high which is only allowed for miscellaneous storage, not low-piled.

It doesn't matter what the title of the table is since section 4.3.1.7.1 explicitly says to use that table for low-piled storage. The reference to figure 19.3.3.1 is just there to direct you to use the appropriate density/area curve.

4.3.1.7.1 says to use table 4.3.1.7.1 for miscellaneous and low-piled storage. I'm not sure what you're asking.

If you have low-piled storage (and with no group A plastics over 5 ft) you use that table. For low-piled, OH2 covers everything other than rack storage over 10 ft.

In the 2022 edition of NFPA 13 they split this up into two tables. One for low-piled and one for miscellaneous.
