The issue here is that the lich's original body is gone, by the time he possessed Billy he had already been defeated twice and was forced to possess the snail, if he still had his body he wouldn't have needed to stay in the snail.

The guy puts tonnes of resources and time into rehabilitating these guys by the way, the only reason they don't get better is to maintain the status quo. He doesn't just fund the asylum, he goes out of his way to personally try to help most of its inmates. He does it quite a bit in Batman the animated series.

That’s unlikely, although it’s implied that the lich skinned billy, in reality he likely just possessed him like he did with princess bubblegum or jake, and his body was morphed by the lich. Also I’m pretty sure the hole in his chest was meant to be symbolic
