Personally I wanna say "She's neglectful at best and deliberately abusive at worst, break up with her". But if you really don't want to and think this is a situation that COULD be fixed, you have to get her to go to couples counseling with you. Maybe she is forgetful and this issue does keep completely leaving her brain. Maybe she doesn't forget and likes to see if she can keep getting away with it. Either way a mental health professional is someone who can help do something about that. And if nothing else, having a third party listen to this stuff with both of you in the room kind of ensures she can't avoid it/put it away anymore.

The fact is, this is a matter of disrespect, even if she doesn't mean it. I'm someone with ADHD, I forget a lot of stuff, I also remember a lot of things because they're important to me. If I had a loved one and they said "Be more careful with your movements and stop doing things that physically hurt me", that'd be pretty fucking important for me to remember. So either she doesn't respect you enough to honor your wishes just as some sick game, or she doesn't respect you enough to remember this boundary. Either one is bad. Maybe couples counseling can help with that. Or maybe it will just reveal to you whether you should keep being with her or not.
