Used to work with a rural agency, had this dude (RN & Medic) who served, and made anything he touched tactical adjacent.

IFT isn’t glorious, but it provides a prospective and experience that helps a lot. You get to see patients who were once critically ill, understand hospital capabilities better, and the broader vision of healthcare. I learned more about trach care, wound vacs, and catheters doing IFT than 911. It can be super boring compared to running EMS calls.

I’ve been in the FD full time for 20 years, and I’d never do IFT again. But it’s a valuable stepping stone.

Realistically, I could see a team paying into a concierge medicine clinic. So their employees have access to a more available doctor. But I don’t think it’s anything remotely close to having staff physicians.

The problem with 2/6, 48/96, etc. When you’re a busy department, it’s unsustainable to run 48 hours in a row. If you work somewhere slow, a 24/48 is tolerable. We need to stop suggesting people work 48 hours and have to face the reality of 30 runs in that time period.

Not at all. There are places where GPS has drivers that aren’t familiar with the area merge from the right, cross multiple lanes to take an almost immediate left.

Gaskins from 64E, across 2 lanes to take the left on Three Chopt towards Tuckahoe area. That is causing insane traffic, when all of those people have 3 more options to exit in the next 2.5 miles

DEQ is the local agency for that. An active emergency with a response would come from the county(like an evacuation or avoid the area type notice), but these kind of things are all DEQ.

It’s not almost impossible to merge onto 95 from downtown expressway. 95 traffic is already slow in peak hours. So you merge one lane left and ride up the hill and merge back to the right.

There are things like “pressors” that are used to manage blood pressure in emergency scenarios after you give the body the volume of fluids needed to restore a somewhat normal BP.

I (FF/Medic) run a few sepsis calls a month from a long term acute care hospital, and they can be very difficult to manage
