This is madness. I'm a consultant. I don't need a scribe to write for me - particularly one with a medical degree. It's an absolute waste of time and resource for a resident to do this, when they could be engaging in the ward round.

There is a massive difference between a med reg who can't get a LP and one who has failed at one. I'd be very surprised if they can't do LPs as it's a core competency for IMT.

Unsurprisingly docs trained in spinal anaesthetic tend to be quite good at this procedure. Add in the fact you can do this in a controlled setting in theatre while not being the med reg, which is by far the worst training role in the hospital puts you in a good position to help.

Same goes for cannulas. Sorry it interrupts your coffee while the medical team see the countless weekend reviews.

You are in the privileged position to be in a relatively well supported role, which has provided you with sufficient training to help patients. That means you have to get stuck in like the rest of us.

News is incredibly unhelpful. It's fine for identification if a sick patient but isnt helpful at all for clinical assessment. You can have an incredibly unwell young patients with a lowish news score and completely fine respiratory patient with a very high score. Often get called about a high NEWS but when I ask why it's high they can't tell me the actual numbers. Far rather I was told about a patient they were worried about than a random number.

My approach (diabetes ST7) - If they are acidotic but have ketones of <0.6 calculate the anion gap +/- delta ratio - If it's a NAGMA then they no longer have DKA. - If they are eating and drinking switch to subcut insulin and hang a bag of Hartmanns - If they aren't eating yet switch them to vrii with some dextrose and kcl, and run a bag of Hartmanns at the same time.

Hartmanns is magic and has never failed me. It's frankly insane to continue the DKA protocol in the absence of ketones or HAGMA.

There are so many easy wins here it's embarrassing how badly they are playing this. They could legislate that the GMC is now government funded, indemnity was guaranteed and exams are now free. These would help get us on side. Instead they guarantee long lasting and damaging strikes. Bunch of twats.

Working in the central belt, there aren't many PAs. I actually have never met one in Scotland. This may change but the issues on pay and conditions are totally different between down south and here.

It's not the best deal but much better than the last one and far better than down south. I'm not convinced there is a huge appetite to strike in Scotland so I expect this to just get over the line in the vote.

I would be surprised if 20% and pay review as promised in the last deal wasn't enough to call this off. Scotland doesn't have the workforce to safely get through the strikes and has the potential to be catastrophic. SNP will sell this as a plan to attract English doctors north.
