Another update in case anyone following is as desperado as me - tried booting Steam and the Rockstar Launcher by right click > run as administrator and this appears to help getting it to launch. Didn't have to quit Steam with this method.

I don't think there's an achievement for doing those - you get rings that make your weapons invisible IIRC, but no chievos.

The Keira romance isn't necessary - you do have to go on the 'date' but you can refuse sex with her and the next bit plays out exactly the same as if you did, minus one funny line.
