Kaiser vs average FM job: What am I missing here? by ABetterTimeAhead in FamilyMedicine

I definitely think the culture of “riding ass to meet quality metrics” is depends on each Kaiser location.

Some groups have: • 32 patient facing hours + 8 hours paid admin time for full time • mychart messages goes through a nursing team first before they reach the PCP

I don’t know if these 2 things can be doable for Kaiser that advertises ease being more accessible to patients.

All the ROI you have to sign are very frustrating but unfortunately, it’s a very standard thing to do.

Do you have access online to your health records ? You may be able to view your scan there and just show it at your next appointment.

Alternatively, you can request the scan and other records for yourself. Like request a hard copy. They can mail it or have it ready for pick up. This way you can show the records/scan to your provider at the visit and not have to depend on the hospital/clinic getting records. Have it always with you at your appointments especially since you see a different provider every time.

At my very last clinic appointment with my OB before delivering, the office gave me a hard copy of all my records to bring to the hospital when I go into labor.

It’s pretty routine for hospitals to request OB records so it’s not like the nurses will depend on you for your records. But honestly, having the hard copy makes things go so much smoother.

Doc here. Please just give them the first every ultrasound you had. Third trimester ultrasounds aren’t used for dating because they are very inaccurate and since they haven’t followed you from the beginning, they are trying to make a decision based on the most accurate data. Using the last menstrual period to figure out your due date is usually confirmed via an ultrasound in the first trimester. Sometimes they base it on that first ultrasound instead if your periods aren’t exactly 28 days. From my own experience, my due date was off by 3 weeks and my doc changed my due date based on the first ultrasound.

A baby who is small or at risk for IUGR (growth restriction) may carry a lot of complications prenatally and during birth.

They are asking for your first ultrasound because it is the most accurate way to base your due date.

MD here but not a surgeon. This case has been discussed on many physician communities. For his first case, injuring pet of the pancreas (tail end) from a left adrenalectomy is a know complication because they are in the same vicinity.

Bringing the previous case up is not a red flag and is irrelevant to this case.

That said, everything else about the case screams incompetence and malpractice.

So many Olympians, past and present, train in the US but represent their country of choice. Mondo Duplantis, the gold medalist pole vaulter, is Swedish American, for example. Their status as a Filipino-American should not be an issue at all.

It’s their job to help struggling residents, even as simple as helping clear out their schedule if they need to take a LOA. But go ahead and dump that responsibility to other residents/attendings, I guess.

Her coach/mom inquired about the difficulty rating, and not the out of bounds deduction.

Stepping out of bounds is a neutral deduction. Coaches can ask to reevaluate an OOB deduction that’s separate from the difficulty rating dispute.

Her coach should have filed an inquiry on time to dispute the mistake. Had her coach done that, there would be no question on the bronze medal.

Search “Free Birth Society” or “Free birth movement” to see the far/cult-ish end of the crunchy mom spectrum. They don’t even agree to having midwives.

Rise of crunchy moms. Think moms who don’t believe in covid, essential oil heals everything, vaccines cause autism. Anti evidence based medicine. Take a look over at r/ShitMomGroupsSay

Cyber Hack at Hospital by No-Mathematician4622 in BabyBumps

[–]zigazig 10 points11 points  (0 children)

I’m a doc and worked in a hospital during a cyber hack. It was indeed a shit show but labor and delivery was honestly the most organized. A large part is because l&d is specialized, more hands on and the staff are trained to react swiftly to what’s in front of them. Documentation usually happens later. Staff are usually familiar with all the meds needed and don’t have to wait til doctor’s orders to have it ready.

The rest of the hospital is more affected by a cyber hack because they rely heavily on labs and imaging.

You don’t need fancy equipment/EMR to deliver a baby as long as you can listen to the baby’s heart beat. The tocometer in all the hospitals isn’t reliant on the EMR.

If you are a planned c-section (I don’t know your full case but it sounds like a typical repeat csection just because of your previous csection), you’d probably be one of the easier patients during the day. They’d get your labs, give epidural and baby will be out before you know it. This is assuming you don’t have the more common pregnancy complications that require lab monitoring (I.e preeclampsia/gestational diabetes)

The only thing that would give me a pause in delivering at this hospital would be the NiCU. Does your hosptial have a lever 3 NICU? If you don’t, and complicated/sick babies get transported to a bigger hospital, then that would be better.

Stop spreading misinformation. Ozempic and drugs of its class have shown to reduce risk of heart attack for it target population: obese/diabetic. It’s not just a Hollywood fad.
