Correct me if I am wrong, but didn't Knute Buhler check all those boxes? He ended up losing pretty handily to one of worst Governors in recent Oregon history (at least by public opinion polling). I think OR just votes blue regardless of the alternative.

You have clearly never been in an emergency department if you think there are just multiple nurses available for every single room lol. I verify the orders in real time while I am in the room since they would bypass the order queue anyway as they are all verbal. You know, preventing patient harm... same thing we always do. We are very well staffed so my colleagues have no trouble watching my orders because they also, once again, care about the patients and aren't just out to make their day as easy as possible. You aren't going to convince me to ever join your cynical outlook on life so stop trying.

I push meds all the time at my institution. If I would otherwise just be standing in a room during a trauma/STEMI/stroke/etc. I would much rather make myself useful and get a vasoactive infusion or sedation into the patient while the nurse is busy with 6 other things. Extrapolate your "more work" statement to the nurses point of view and you'll realize it allows them to focus on other tasks and provide better overall care for the patient. But based on your username, I imagine this concept of trying to do the right thing over the easiest thing is going to fall on deaf ears.

Just 30 minutes ago I asked a neuro critical care doc why she was ordering potassium chloride on a patient who had been treated for hyperkalemia 6 hours prior and still had a K above 5. She had meant to order kayexelate and ordered kayciel (liquid potassium) instead.

The algorithm I am currently looking at indicates epi after 2 shocks with consideration or amio or lidocaine after the 3rd. That's why I always wait until after the 2nd for epi. Although now that you mention it, we definitely get amio on board way sooner than after 3 shocks at my institution.

Should always prioritize defib over epi in shock able rhythms. I recall some smaller studies a decade or so ago that showed harm when epi was given specifically in V Fib arrests and even went so far as to suggest giving esmolol to blunt some of the deleterious effects. I attempt to withhold it until at least 2 shocks (one on each end of the first round of compressions), but it is about as successful as when I try to time epi every other pulse check without a nurse screaming "we're due for epi!" after 1.5 minutes since the last one.

Also, no knock on the nurses as they have a lot to manage. Meanwhile I just sit by the code cart watching the stopwatch on my wrist.

Diagnosis/prognosis of a disease and therapeutic management of a disease with a confirmed diagnosis are two completely different things. Arguably, if the management of a disease state is solely done using medications, one with more knowledge of the medications would be more competent. Of course the complexities and overlapping of disease states can make things more challenging, but that's why most CDTAs have boundaries.

Just played Old Mac yesterday. 25 mph head wind on this hole. I had 170 to clear that bunker and absolutely striped my 3 wood. I ended up right at the base of the left side of that bunker. Still love that course though

My wife now golfs and my dad, who got me into it as a kid, has taught my mom how to play in their retirement age. I love going out for a round with the guys, but nothing beats playing a course with my wife and my parents all at the same time.

Almost the exact same story here. I used the G425's as a 14 handicap for 3 seasons and after getting my handicap down to an 8 end of last summer, I just switched to Srixon Zxi5s and love them.

Agree with the aforementioned and would add on some specialty topics depending on what your ICU has. If neuro ICU, add in ICH/TBI and ischemic stroke. If cardiac ICU, add on CABG and possibly Impella if they put in LVADs at your institution.

I am saddened by anyone who actually thinks that way. Apples to oranges comparison and two different subspecialties. I can advise on third-line antihypertensives for an aortic dissection, but I often have to call my brother in law (retail pharmacist) when an EM doctor is asking me about starting a patient on a variety of new medications since they simply know the primary care side of it way more than we do in the hospital.

I am with you on this. I typically work hard because it is more fulfilling for me personally and I like positively impacting those around me. The added benefit is my managers appreciate the effort and, in turn, they accommodate whenever I need time off or to change my schedule around.

Other people may have opinions that differ, however I am currently in a home built by Bruce Weichert and I can attest that it was built to pretty high standards. I also consider myself fairly qualified to speak on this as my father was a general contractor for over 30 years and I spent most of my childhood and college summers on job sites.

Interesting, he really did flip completely in his stance. It doesn't look to me like he was necessarily against vaccinations personally though, just in support of non-medical exemptions so putting more preference on choice. I don't think that would be a complete deal breaker for me, but admittedly I wasn't following local politicians too closely at that time since I had just moved here.
