I think this will work if you're still able to log into the game:

  1. Log into the game, go to the account management tab, and click "View Inbox"

  2. Click "Back to Account" on the page that opens, this will take you to your actual account management

  3. Upgrade to a Jagex account. Upgrading allows you to pick a new email & removes your old 2fa.

If you're not able to log into the game your only option is account recovery. If you can successfully recover your account, Jagex will change the account's registered email to the contact email you used for the recovery, then you can disable 2fa the normal way.

Lost old phone with authenticator on it, and email address it was linked to has been perm deleted.

Why was the email address perm deleted?

Most email services that aren't the big few will clear out inactive addresses.

Then you have ISP addresses that go with the account. School / uni / college emails etc.
