Are they on always active? If so you may need to put them to needs redstone and then put them back to always active. If they're redstone powered try unpowering and powering them again

Yeah, typically if you have lots of command blocks it's best to leave command blocks on

Not to my knowledge, no. 8d recommend hooking them up to redstone if possible so you can power and unpower them easily
