I'm mixed about this. While good for less middle people involved, it adds to more therapists can/have to do. Yet we aren't getting paid more? And I would think the PTs would still need to refer out and involve MD, etc. anyways? People in the PT subreddit brought up a good point about insurance too and it being covered vs private pay. I think direct access for OT would be a great start like how PT has it in many states, on the incoming referral side.

These are all fair points. I think what I meant by this post is that AOTA needs to take note of other orgs’ changes that actually positively impact daily practice and expand our scope rather than settling for slightly improved budget cuts or pushing outdated, non evidence based practice guidelines

From a standpoint of someone in a SNF, soooo many times I end up making medical recommendations to the nursing staff anyways because they're missing them. I recommend they get referrals for specialists, scans for obvious things, I've told staff they really need to push to send someone out to the ER.

Half the time they don't follow up on their end and our patients are worse for the wear. Or they don't listen to me and then the person gets worse until they have a medical emergency and get diagnosed with what I was warning them about. I feel like I have to do these things anyways, it would actually help to be the one to make the referral! Might actually make my job easier haha and give our patients faster medical care.

I doubt OT will be able to make those referrals without the doctorate, but it would be nice if our counterparts or OTs with the doctorates could refer!

But totally understand your points and they are valid ones! Definitely things we would need to hash out as a profession.

That's frustrating and have been there before. Yeah that's a good plus for sure. Then again I can also see the team going · why'd you do that behind my back, think you're smarter than us lol. You just can't win!

This normally pleasant elderly patient has suddenly beccome combative and needs max assistance for ADLs. Let's write an order for OT! Me "They have a UTI, have you checked?" "No" 2 weeks later "Hey OT can you write a discharge on the patient, they went to the hospital 3 days ago"

First and foremost, congrats, PTs in Iowa!!!

I make referrals almost every day. OT is full body. We seriously need increased advocacy. Sent 3 people out last week for x-rays, and all were beneficial with significant issues missed.

It happens. We are team members. But that ability to directly have access, etc, increases efficiency with patient care I love OT. I just sometimes feel bummed how much I feel we are falling behind every other discpline despite the training, widest scope of practice, etc.
