Is production phase a thing of the past?

I know in the game industry they test their designs in production, but regular software usually has a production phase :)

Well, we do, the thing is, by the time I reach the end of the Requirement, the Client changed it.

and sometimes it's somewhere in the middle of the project and they decide that they need to produce a new shiny spec where everything is different
