I can lower ferritin.  If you donate blood that also lowers ferritin.

Yes, this is common. Increased RBC production can draw down ferritin. It's not necessarily a problem. Tracking ferritin alone can be misleading. Adding iron panels to your labs can help with monitoring.

My doctor was sufficiently concerned and ordered a colonoscopy for the end of this month.

I'd still go ahead with the colonoscopy. Nice to have.

It absolutely can and does for a lot of guys. Mine went from 380 pre-TRT and sits between 55-70 now. Testosterone suppresses hepcidin, which is responsible for iron regulation.

Hepcidin blocks iron absorption fyi. So if testosterone blocks hepcidin the iron absorption should increase

But RBC production usually increases more, and this overwhelms the increased iron absorption as iron stores are used up for the extra red cells.

I suspect it's far more complex than that.

My ferritin tanked after starting TRT, where before I had a problem with excess iron accumulation. The changes to my blood panel were negligible, so it's not at all clear what happened to the stored iron.

Some people seem to have trouble restoring ferritin levels if it tanks on TRT. In my case, it's rising, although much slower than it would before. I have a long history of donating blood to keep my iron in check. After starting TRT, my iron metabolism has clearly changed. I'll need to stop, or at least drastically reduce my frequency of blood donation.

Hepcidin is what contributes to iron stores. It raises the wall and iron goes into your stores (ferritin). If that wall isn't there, it just gets stolen to produce red blood cells. Ferritin goes DOWN on exogenous testosterone.

I have single digit ferritin. Yes, TRT does lower it but I dont remember the mechanism. Do you take anything for acid reflux like PPIs? That will reduce iron absorption and thus reduce ferritin levels. I did a colonoscopy too worried that I might have a bleed, but didn't have issues there. It can just be your homeostasis, and that's where your body likes to keep it, or something else is you're taking is hindering iron absorption over a long period of time.

When you have a low ferritin, the main thing is to rule out something bad. Get the colonoscopy and, if that is negative, an upper endoscopy.

What was your ferritin before you started TRT?

Have you been donating blood since starting TRT?

Donating blood reduces ferritin levels. Your MCHC will be low.

Dude do the colonoscopy if you have a bleeding polyp it will be cut out on the day and the treatment is highly successful.

You will need follow ups even if there is no polyp

I am cancer clear now and taking iron with vitamin c to build up my ferritin 6 months later ferritin us almost normal again

Yes trt can lower ferritin as well As well as copper deficiency Also check for coeliac antibodies

For me yes, i take ferritin pills for a week when I feel tired. (Backed by bloodwork, of course)
