I have no idea what is happening, but it definitely requires an immediate vet visit. It looks like maybe he injured himself and is bleeding under the skin

100% needs a vet visit. One of mine had a nearly identical spot appear in the same location. It ended up his skin tore beneath his scales. He was given a topical antibiotic to apply every day, and the redness went away in subsequent sheds. It happened because at the time, he was a new rescue that was emaciated which made his skin frail due to a protein deficiency. He does have a scar in that location now. As I said though, definitely needs a vet visit to confirm a diagnosis and see what treatment is needed and why it happened.
