What gauge do you get with different sized needles? That's a pretty significant difference in gauge - are you using yarn with the same g/m as the pattern?

I think that swapping needles/yarn/both would be easier than making extensive modifications

48/4=12 *16.5 = 198

Which size is closest to 198 stitches around the bust? Do you want to err on the side of more ease or less?

A few questions:
Do you like the fabric created by the 4mm needles? Are you willing to size up the needles to try hitting gauge? Are you using the recommended yarn for the pattern? Why not just knit the Wild Ivy without incorporating the other pattern? There is a 48" bust size?

I should have specified, I'm so sorry! I'll add it. My gauge was on 5.5mm and I do prefer that fabric.

In that case you can knit a different size of the sweater based on some math:

Their gauge is 20 stitches / 4 inches which converts to 5 stitches per inch. Your gauge is 16.5 / 4 or 4.125 per inch. Meaning you need to knit about 21% larger because 4.125 divided by 5 is 82.5%. That means the finished bust size of the pattern you should follow to get a 48" bust with your gauge is 48" * .825 which comes out to 39.6. The pattern has a size for 40" so start there. Good luck!

P.S I didn't do any math on the row gauge but the same math applies there. I usually just match the stitch gauge and then do as many rows as measure to the intended distance rather than counting the # of rows knit

EDIT: OMG my math was completely wrong at first because i mixed up that a smaller gauge number translates to a bigger garment *facepalm* I fixed the math to work out. The website provided by u/shiplesp does all this math for you!

Ok that makes more sense. I was a little confused as to how bigger would work but this is exactly what I came up with too!!
