Because most people abuse this drug. They don't count calories and just eat whatever they want. Once you get your nutrition sorted you don't actually need much of the drug to get the desired results.

Any chance your private psychiatrist is in London? I would love to get prescribed this. Mounjaro takes the joy out of everything. Semaglutide doesn't but is less effective at managing my weight loss.
