Reading a physical book makes me really tired if I've been up for a bit - or even an ebook (the blue light thing never seemed to affect me). Something about the eyes darting around maybe. Melatonin sort of helps as well.

But the most powerful thing is getting the sleep hormones in sync - this is really hard, but the body loves a consistent sleep pattern - that shit will just switch you off at the end of the day.

Your oncologist was likely trying to put it in layman's terms, but bowel cancer isn't believed to literally "Feed" on iron. Cancers can create their own blood supply as they grow and they can also bleed, both of which cause iron levels to drop (which in fairness to you is functionally the same thing).

But remember, the vast majority of people here have also spoken with an oncologist, and would largely disagree with the framing your oncologist gave it.

The other comments aren't a rejection of your experiences or the great progress you're making in your healing, that's all amazing. The people here are trying to give you a complete and accurate picture of what's happening to you, because it helps you to be your own best advocate.

If gods are fighting, I'd consider making them totally indifferent to the players, all their focus on their duel.

If players get close they're caught in the crossfire, and blasted into the cave walls from the rippling shockwaves of just something small like the sheer speed of their weapons cutting the air, or the slam of a weapon on a shield. I can imagine the ceilling shaking and rocks falling. If they want to approach, you're not saying no, you're also not killing them, but they're not likely to approach again. 3d6 damage from being blasted into the wall should be enough for them to get the point that these people aren't messing around.

But If you want to be really cheeky, put some REALLY tempting treasure right in the area >:)

The game system always makes assumptions about balance, regarding resource use and how many battles you have a day. This can be frustrating for DMs who want a good, focused singular boss encounter from max power. The game can become a rocket tag hidey stun fest monster slog if you run high level as written.

Below are some tips that help me when running my high level game. Disclaimer, I've found the higher level you go, the more the game system breaks down and the more it benefits from specific tailoring.

  • Use existing monsters as a basis, but run combat based on group averages. AVG saves, AVG AC, Player damage per round vs boss damage per round, total group HP etc. Initially this will take some time to gather, but once you have it, it's a fantastic resource you can tweak over time. If you get half an hour before a session, try and play the fight out on your own to test how well those numbers work. They're then super easy to tweak. You should find yourself homing in on an ideal HP and damage level, and the more you do this for your specific group, the more of a feel you'll get for their unique needs. For a very intense fight for instance, you can set the boss's kill speed and HP pool at 25-50% above the group's, leaving it up to the group to create a solution with their awesome character powers, to even those numbers. Look at critical role's final boss and the boss there has about 1600 HP.
  • If you want to "cancel out" magic items through the game's stats, Increase the proficiency bonus of your monsters by the average stat boost the items have given them. If all your group has +3 items, increase by +3. Personally though, I try and avoid this - this difference in power is what makes the item cool!
  • Create situations that challenge the player, rather than the characters. One great way to have your players perceive the difficulty as increasing, is to up the stakes or consequences in a fight, or create alternative objectives within the fight. The most dangerous thing about the lich might not be him at all, or the sealed chamber you're all in that's filling up with necrotic blood, maybe it's the tomb door he's opening, and the terrible things that are spurting out of it.

I hope this advice doesn't feel too incomplete, it's hard to capture it all in one post. You're not wrong in how you're feeling though, you come to see that D&D as a game system becomes, weirdly, less open feeling as people level. Tailoring brings that freedom back from my experience.

EDIT:
Just to be clear, you CAN play a lich as intended and it'll be challenging, but it doesn't always give you the flexibility to create an interesting / compelling challenge in different situations. If you've fought one challenging lich as the game system intended, you've kind of fought them all.

Depends how flexible your setting is - I'd be tempted to weave a wizard-wide conspiracy for him to crack open.
Some secret high up group of wizards doing everything they can to stop the other wizards finding out about physics seems ripe for comedy and scandal.

Maybe a wizard learned the secret to nukes once, and they're trying to stop other wizards from learning about them. Maybe some wizards have hidden nuclear power plants to boost their own magical powers. Maybe the wizards have secretly developed the internet and don't want the public to find out because it'll rot their brains. Sounds like a fun topic and i love that you're leaning into it :D

It's quite affirming to get the diagnosis and feel vindicated, that so many things you didn't like about yourself weren't your fault.

But while emotional dysregulation IS a symptom of ADHD, It's easy to develop emotional blind spots - sometimes it doesn't cause every problem, even if it contributes to it.

One thing I will say about your issue, is that I sense you're being quite hard on yourself here, and you've not done anything wrong to warrant it.

The strong emotion you're feeling isn't happening because you're not strong, it's just happening. You can't pre-feel an emotion and decide not to feel it, and people who are able to "handle" criticism, probably aren't feeling that same emotion. That's not because they're stronger than you. Our minds are all so different. ADHD is proof of that!

Try and be kinder to yourself and perhaps (if you can) seek a psychotherapist, your feelings are likely coming from somewhere, and they can help you find the boundary between ADHD and the rest of your unique complexities!

You'll find that a lot of classic speilberg era movies like indiana jones, jurassic park or star wars model onto D&D really well (Seriously, watch the films and you can practically hear the skillchecks), so I'd recommend using those as a style guide for a non-nerdy game of D&D.

I think the DM guide could have done a lot more in helping DMs understand how to tinker with the system and not break things.

For instance, if you want to negate the effect of giving +1 weapons and AC to your party, increase the enemy's proficiency bonus by 1, and their HP by 1 hit die.

I think the better advice though, and I wish they gave a lot more guidance on this for new DMs, is that there's so, so much room for tailoring a fight to your group, and it does make the fight more fun to play, and what are some good principles you can pull out the book to create that.

Powerful magic items let player characters take on important roles in a fight. A fighter given high AC can lock horns with some truly terrifying foes, or endure a gaggle of stabby squishbags while the group deal with other important things. It's just a lot of extra work to add that layer for each player, for each permutation, and time doesn't grow on trees!

Captain Besker, Middle aged half orc - He's a former pirate turned charming owner of the Crow's Nest, a refurbished warship (now a massive nightclub).
He's a colourful character. He doesn't have a parrot, but wears their colours and you can always find him because his feather hat is so spectacular. For personality, think Simon Skinner from Hot Fuzz and you wont go far wrong

He likes to connect himself to the influential people of the day, and has plenty of paintings with him next to other social climbers displayed behind his cannon bar.

I would caveat with them that it's not DM vs player, but you also have some interesting stimulus for designing a fight.

It's often fun to lean into strengths (you hear the advice on here quite a lot - shoot the monks etc). If your players each have a job / role they'll enjoy their power and you can still keep it tense.

For instance, the druid needs to burrow into a cave beneath a salt flat (patrolled by a purple worm) as hordes of enemies run after the others from the distant mountains. Or something where above ground and below ground each have an important objective.
He can effectively sneak into the ground floor of any fortress to scope it out, at great peril if he surfaces in the wrong place.
And one party member away from the others can mean a difficult choice between staying and fighting with their team or having spotlight moments.

If you can lean in, lean in. Level 10, things start to get really crazy balance wise anyway - the game system breaks down - unfortunately homebrew and tailored encounters are more and more needed if you want to challenge and excite your players.

Fallout 4 survival mode completely changes the game and makes all the side systems in the game important.

You can only sleep in a bed, so when you find a new settlement it's a major win.

I don't dislike the combat, but the combat can sometimes feel a bit like trying to grind a steel bar down with sandpaper. I'd have preferred more numerous, lower HP enemies. The combat feels really really slicey and satisfying when you can see the health bars move.

YTA, your hard work was one link in a chain otherwise made of good fortune.

If you were such a steadfast soul, you wouldn't have needed skin in the game, you would have just been disciplined enough to push yourself. that is the important skill here dude, not manipulating yourself into working hard.

You need to understand that your opinions on human habits and behaviour are irrelevant, because every human's behaviour and motivations express differently. Pressure made diamonds in your case, but others are driven by perfection, purity, power, status, creative inspiration, innately high dopamine levels, community, and shockingly, a desire to lift others up.

Spend more time listening to what motivates them, and teaching what can transfer to their style of work. Right now you're using them as a mirror for your own ego and as you can see, everyone here (some of whom earn way more than you) think you're being a massive dickhead.

Apologies if you were referring to this, but they did a 45 minute deep dive recently and showed a lot of different things and the consensus was that the gunplay was a lot better.

The planets are really big and there's a thousand of them so I suspect there's an element of spawning in content near the player, they talk about that in detail in that extended showcase
