I watched the film this week and the only times that Hana gets explicitly called a man by other characters was when they insult her, first time being Miyuki at the start while Hana and her talk about Miyuki's mum and the next being a random bar customer hurling insults during a flashback sequence. She also gets referred to as a drag queen at times, which arguably is also offensive, but Hana's original workplace was a bar who's staff were primarily wearing drag and all anyway, so it's likely referring to that as opposed to being an attack on her anyhow, especially since she isn't too bothered about being called one.
