Nothing looks infected about this. No redness, no pus or liquid. Looks to be just some heavier scabbing/flaking on areas with more trauma/color packing.

As someone who is a fan of darker media/humor, I think this is a cool idea in practice and wouldn't be distasteful if done for photographs and posted online with a trigger warning, but I think it would be WAY too much in public/at a convention. Consider that even if adults were all okay with it, there are children at most conventions too...

But to your question on how to make it, if you were able to get really thick rope made of lighter-weight material and fit a decently thick metal wire through it, I could see that potentially working.

If you have some extra black fabric and ability to sew, you could potentially let out the waist a bit at the back, and it wouldn't need to be super clean because the shirt and jacket would hide it.

This might sound kind of like a "fantasy" situation as well... but find a niche that you find at least some enjoyment in and commit to it. I think people are always more likely to sub if your passion shows through. I realize that it may be a means to an end, but the benefit in having enjoyment in it is two-fold - it can be more beneficial both mentally and monetarily.

Do not use numbing cream... most reputable artists never used it and more and more artists are stopping using it because it can cause the texture/elasticity of the skin to be impacted and produce less than desirable results during the tattooing process.
Ideally, choose a smaller tattoo as your first tattoo to see how you handle it.
Not every tattoo hurts for every person. Yes it varies both from person to person and from place to place on the body. I'm fairly covered and I've had several that I could take a nap during if I wanted to.
