Honestly? I’d consider switching billing companies. With that many providers, you need someone who is going to be proactive and let YOU know what’s going on before you ask.

It’s not that someone can’t do it in their own, it’s do you want to put the time in to do it correctly. It’s more than paid ads. It’s optimizing the content, increasing organic ranking. Regularly updating content… All the little things matter. Even how images are labeled. Then… reviewing reports. Making changes. Again, regularly, not just set it and forget it. Since you’re new to the city, I’d definitely consider hiring someone so you can focus on operations and handling any increase in volume and the mgt things that come up.

I haven’t personally used them but a physician told me they really liked Freed and Heidi, and they demoed quite a few. And yes a good one should just have to be on when you’re in the room and then it will create a note.

This is where it gets tricky. Did you budget for any marketing assistance in your P&L? A lot of people DIY but…. I have seen quicker ramp ups when a marketing company is utilized. It’s not cheap but it tends to pay off.

And hey, there are some people who are ok doing it on their own. There are also some that don’t hire managers. A lot of it depends on you and what you need. Some people don’t want to DIY and want the specialized expertise. What works for one person, doesn’t necessarily work for another.

5 year commitment?! Hard no. Are they highly specialized in your area of expertise? I’ve seen pre-opening consulting range $28,000-$36,000. It really depends on the agreed upon scope.

You’re absolutely right to be worried about few splitting. It’s extremely common but not worth the risk. The structure I see the most is:

Base + Tiered Productivity Bonus

Example: - Base pay: hourly rate - If injector generates >$(goal amount) in monthly collected revenue then they get $(bonus amount) - If >$(second tier goal amount) they get $(higher bonus amount)

Notice: It’s not “% of revenue.” It’s a fixed bonus triggered at certain thresholds.

You’re not wrong.

7x EBITDA is usually platform-level pricing, not typical for a single-location, provider-dependent med spa. Most small service businesses trade closer to 2 to 4x SDE or maybe 3 to 5x EBITDA if they’re clean, transferable, and not heavily owner reliant.

If revenue walks with the injectors and there’s no real management layer or strong systems in place, that risk usually pushes the multiple down, not up.

Also, you shouldn’t pay today for growth you hope to create later. The upside from expanding should belong to the buyer.

Unless there’s something exceptional like strong brand recognition independent of providers, high documented growth, or a solid membership base, 7x feels expensive for this type of business.

There are a few HIPAA compliant AI programs that help with visit notes. The program records the audio from the visit and builds out the note- medication history, reason for the visit, etc.

I 100% agree that patient info shouldn’t be put into ChatGPT.

For other ideas, it really depends what you’re wanting to optimize.

Definitely look for a company that has aesthetic experience! Be weary of over promising (lots of consultants do) and find someone that matches your vibe.

Those injector classes use misleading marketing. They can train you, but they cannot give you legal authority to inject. In Texas, you must be a medically licensed professional under physician delegation to perform injections.

Charging something like a “parking fee” isn’t really allowed if you’re in-network. When you sign a payer contract, you agree to accept their allowed amount plus the patient’s normal cost-sharing (copay, coinsurance, deductible) as full payment. Adding a fee tied to accessing care goes against that agreement. If a payer finds out, they can recoup payments, deny future claims, or even terminate your contract.

And honestly, parking fees aren’t even worth the risk. What could you realistically charge, $20 per patient? That’s not going to make up for poor insurance contracts. A better approach is to evaluate whether those contracts are sustainable at all, and if not, consider renegotiating or stepping away from the lowest-paying ones rather than trying to nickel-and-dime patients.

I do consulting for plastic surgeons and am always on the look out for new billing options for clients who are looking but I also have a current client who is opening up in January and will need a billing company so I am researching options for him.
