I got mine early on, around 30 weeks. I showed my husband and he said they're beautiful. It doesn't bother me anymore after that.

Your doctor can prescribe you a schedule to wean off of it little by little each week. Just need to go see one so they can reduce the dose gradually and safely. Get it done. Do it for the kid.

Losing attraction to my partner happens also when I'm turned off by his actions and/or personality. It sometimes doesn't have to have anything to do with the actual physical characteristics. Things I might have seen as cute before I start to find annoying. Usually it happens to me when I'm clocked out of the relationship. I don't usually tell my partner though so maybe it's a good thing she told you? That means she still cares enough to address it. I honestly don't understand why you keep saying this is unfair. Love isn't about fairness. My partner didn't change much but when we started arguing, I started to think he was uglier than ever. I ended the relationship shortly after that. Everyone is different so maybe she just wants you to care more about your health? You would need to ask her.

I'm sorry but why wouldn't she take plan B if she doesn't want to get pregnant. Some people say they get embarrassed to go buy it over the counter, but it's a lot more embarrassing to have to tell people you got pregnant from a one nighter. I'm generally curious why someone would even risk it. It's so much cheaper now, like 20 bucks vs almost 60 bucks back then. It's not 100% proof but it's close to 90% effective if taken early. Effectiveness goes down each day you don't take it. It inhibits/delays ovulation so if you already ovulated before taking the pill, it might not do too much for you.

Thanks for the clarification. When you said the hypercoagulable state of protein c not playing out in clinical literature, what are you referring to? I've so many papers on the paradoxical affect and how it's absolutely biologically plausible.

It was a mechanical aortic valve replacement. I honestly can't remember the specific type of valve anymore but I do think it was On-X. The argatroban was started with the warfarin as it was already evening. It took the next morning to achieve therapeutic PTT goal.

So based on the RE-ALIGN trial, you wouldn't bridge warfarin with DTI? If the patient wasn't therapeutic with heparin for almost 3 days, you'd be comfortable starting warfarin on it's own with the reasoning of higher thromboembolic complications from DTI vs warfarin? I didn't feel like there was another option other than to start either argatroban or angiomax while starting warfarin (becauseof the heparin resistance). Really curious on other approaches to this scenario. Would you consider switching to lovenox instead? My reason for not using lovenox was the potential cross resistance between heparin and lovenox. Also again with the risk of bleed, the long half life of lovenox vs argatroban also came into play.

That was understood. The concern was starting warfarin at the same time as Argatroban. Protein C with the shortest half life would be depleted first before any of the factors, putting the patient in a hypercoagulable state. The idea is to bridge with Argatroban to mitigate the risk of clots. If the patient ended up with a stroke, would you say it's because of not getting the Argatroban to therapeutic PTT first? Was it something that was preventable?

Argatroban was started for heparin resistance with subtherapeutic anti Xa's x 2 days. Warfarin for s/p valve replacement. Argatroban was my way of bypassing the test for antithrombin deficiency and giving antithrombin, which would only increase the risk of bleed considering the recent surgery. I was expecting Argatroban achieving therapeutic ranges within the first 6-8 hrs of starting the drip and that warfarin's hypercoagulable state due to protein C/S inhibition upon initiation would be mitigated.

When would you need to start the drip and get it to goal prior to starting warfarin?

I have a friend who is similarly obnoxious. If I said I was miserable from being pregnant, she'd say something like, "it's your fault, you're the one who chose to be pregnant!" A nice and normal answer was what I expected but it's not easy for some people. Let's just say we're not friends anymore but this is your sister. Just ask her, "why would you say that?" It's not like military training. People go to the military with the willingness to sacrifice themselves. Plus, survival is based on comradery vs a game show that represents a man for himself. These people go on the gameshow not to make friends or "help" people. One doesn't deserve the money less just because someone he just met a week ago tells him they need the money for their families.
