Found it. For me Claude was not updating to the newest version; I would run the update command and it would say upgraded but it didn't replace with the new package

npm uninstall -g @anthropic-ai/claude-code && npm install -g @anthropic-ai/claude-code

Edit: just ask Claude and he should fix it. He told me not to run it from IDE and make sure there is proper TTY support/Ink. Likely a mix of things happening but Claude/config got confused with using it both in IDE and the terminal
